[Question #2940] Recurring discomfort in penis in last seven weeks

I had protected sex with a high risk CSW on 23rd Sep. Nothing unprotected except touching her genitals. Few days before the sex I had not kept a good hygiene and had masturbated frequently. The sex was very vigorous with deep kissing. After the act, I washed my penis with a detergent used to wash the cloths. I can't say for sure neither can I deny if the discomfort in my penis started right after the act or next day or after couple of days. But on sixth day, the discomfort increased with an increase in urination and constant pain at the tip along with burning sensation and increase in urge to urinate. The doctor put me on cephalosporins course for 5 days starting after a week of the exposure. He also asked me to take Alkalizer until the burning stops and do a STD panel test. The symptoms seemed to have resolved over few days. In the meantime, my STD panel report came negative for HIV, Syphilis, Chlamydia for blood sample collected at 14 days after the act. Urine routine and culture came negative. A Sonography report shows normal prostate and other organs but tiny renal calculi in left kidney and a possible urethritis however doctor said the report is normal. At 28 days, I tested for HSV Igg and tested positive for HSV 1 = 3.15 and negative for HSV 2 = 0.07. Enter the 4th week, I had fever, chills, soar throat, sensitive mouth to food and fluid filled blisters in mouth below tongue, n left cheek (inside). These symptoms seemed to have resolved after taking general medicines for fever and cold. However, my main symptoms of unexplained discomfort in the penis, increase urination, mild pain at the penis tip keep recurring after few days. Doctor advised me to take plenty of water but it seems like taking a lot of water is aggravating my problem. And this happens mostly at day time. Did urine routine and culture again and tested negative. Right now I am on Levofloxacin 500 course for 5 days twice a day and paracetamol 500 thrice a day. I don't know if it is working. Should I take HIV and HSV tests at 7 weeks?

Welcome to the Forum. I'll try to help. The sexual encounter you describe was virtually no risk. Protected sex is safe sex as long as the condom is worn throughout the encounter and does not break. Similarly, touching her genitals would not lead to infection. From your description I wonder if the combination of the effect of strong soap and repeated masturbation might have led to the discomfort you describe. Although blood tests are no recommended for diagnosis of chlamydia, your blood tests do show that you do not have HIV or syphilis although if these tests were taken less than a month after your encounter, the results will not be definitive. Testing for these infections with a repeat blood test at this time however will be definitive.
It does sound to me as though you had a cold or non-STI viral infection unrelated to the sexual encounter you have described.
Finally, the antibiotics you have taken at likely to have controlled the most common curable STIs, if you were even infected at all.

I am feeling a lot better now. However, inside my penis I feel friction like the opposite walls of Urethra are rubbing against each other when I am sexually excited. The erection has mild pain/ discomfort. I also get shooting pain in my rectum out of nowhere. Sometimes, it accompanies the erection other times it just occurs. I have been sexually active for many years with CSW but always used protection and never indulged in oral sex. Now, I want to marry and be in a monogamous relationship forever, I have few questions.
1. What all tests above are not conclusive?
2. My biggest worry is that I have some serious strain of an HPV which I will pass to my future wife because it is the most common STD which can be passed by hand-to-genital contact and that means I was not protected from it. Also, my times I rubbed my bare penis on these women buttocks, between buttocks, lower thighs, belly, etc till ejaculation that is a lot riskier than hand-to-genital contacts IMO. How do I make sure I don't transmit it to my partner if I have it? Is there any diagnosis available for me to test if I am infected?
3. Do you think I had a wart inside my urethra when my symptoms were at their peak and it is gone now?
4. It is strange to me that both times my HSV-2 test came exact 0.07!! Is there any data on percentage of people that are not HSV-2 seroconverted at 12 weeks because of their primary infection HSV-1? I read somewhere that people with primary HSV-1 infection may take up to 6 months to seroconvert !!
5. I am HSV-1 positive, does it mean that I can't kiss my future children anywhere on face? Please give me some suggestions to prevent the transmission of infection to my future children.

You have not followed our advice or the Forum Guidelines. We routinely keep question threads open for up to 30 days after the last questions for the purpose of follow-up. You exceeded that and your thread was closed. We have re-opened the thread for your additional follow-up.
1. As i told you in my original response, repeat testing at that time would be definitive testing for those STIs which were tested for appropriately. Thus, once again, you can be confident that you did not acquire HIV, syphilis, or hepatitis B or C. As you were also told earlier, blood tests for chlamydia are not recommended and so the blood tests for chlamydia add no useful information but, as you were told as well, the antibiotics you took without knowing if you were infected or not would have cured chlamydia if present. Blood test for chlamydia are typically positive within 12 weeks of acquisition of infection so your herpes blood tests are strong evidence 9as is the lask of suggestive symptoms) that you did not acquire genital herpes in the safe sex exposure you described.
2. Please read the many other comments on HPV found on this Forum for more information on HPV. HPV is a nuisance, not a serious infection and as long as you and your future wife follow recommended health screening advice HPV should not be a concern. Most sexually active persons have HPV and suffer not meaningful consequences. There are no recommended tests for HPV in men.
3. No, intra urethral warts are very rare and asymptomatic.
4. Your HSV test results are about as low as such tests can read. I interpret your results as strong evidence that you do not have HSV-1.
5. The majority of adults have HSV-1, often acquired in childhood, occasionally as genital infections. Most of these infections are asymptomatic. If you get cold sores, I would advise against kissing your child or anyone else while the cold sore is present because the infection can be transmitted at hat time. Otherwise, despite the low risk of infection, I feel it is better to show affection for your child than to not contact them for fear of transmitting HSV-1.
