[Question #2961] Hiv risk

Hello doctors, i have to ongoing issues id like to see if you can address. I am a 28 year old heterosexual male from Texas. Previously married. Routinely test, tested february. In march i was in the process of my separation from my previous wife and i got received unprotexted oral from some women i know. About 20 days later i woke up one day and i felt a full bladder, i went to pee and peed very little tho next four days were the same, no stinging no discharge just constant urge to pee. Got a urine test they found rleukocytes and moderate bacteria. I proceeded to get a urine culture and they found over 100,000 units of e coli. They did a US of my bladder and prostate and stated it seemed normal. What would cause a uti in an otherwise healthy 28 year old male? I know it is very rare, i spend a lot of time wading in warm water fly fishing and sometimes pee in the water, also the night before it started i had been holding my urine as to not get up from bed...could these be causes?


The next issue is ive been dating my new girlfriend for 5 months. Weve spoken about her previous sex life and although i think she mightve been somewhat promiscous she says that she hadnthad sex for 3 months when she met me and was selective about who she had unprotected sex with. My issue is when we met she was having very strong abdominal pain and passed a blood clot and she figured she was having a miscarriage from her ex boyfriend...my issue is we are having unprotected sex now and have done so about 12 times the problem is the first times i made her bleed. These last times we were having sex and she started menstruating we were in the shower and last night we were a bit drunk and had sex twice, so all in all my penis has come in contact with her blood a few times in vast quantities... i have not developed std symptoms over the week but i am terrified of hiv. I had never had unprotected sex other than with my ex wife. What do you think are my odds of contracting hiv we have been having unprotected sex nonstop id say about 20 times, since she is my serious girlfriend.


To add on i noticed that on both upper quadrants of each buttock i developed red bumps... about 5 inches in diameter circular they look very red and hurt if i scratch them i also have some on my upper back but alot less and on the back of my arms i dont know of they are folliculitis or ars...it wouldve been about 10 th days after exposure,  but couldve possibly been there before, since i had already noticed a few breakouts on my arms i have no fever or other symptoms. So i went to a doctor and said it looked very much like folliculitis on each side. I let it go and now 2 days ago i developed a strong nasal congestion, painful cough with phlegm, my tonsils don't hurt, and yesterday in a cold room i was covered in blankets and i spent the whole night drenched in sweat. I dont know if the night sweats are part of my cold or ars or just the fact that i was anxious about it last night. i have not developed a strong fever, or rash on my trunk face arms whatsoever. I am currently at the 16 day mark of first exposure

For example at this precise moment i feel fine all i have is  congestion and cough no fever no malaise or fatigue just the night sweats worry me

Welcome to the Forum.  I'll be glad to comment.  I will comment on your specific symptoms in just a moment but my overall sense is that you sound a bit nervous about the possibility that your partner may have HIV.  I suspect the likelihood that this is the case is VERY low (HIV infections in women are quite rare) and would not be worried but have a concrete suggestion which you may find helpful that I will make after my immediate comments.

Specifically, none of the symptoms you have had or things that you describe in your girlfriend suggest HIV.  In your case, while UTIs are uncommon among men of your age, that is what it sounds like you had.  Unless it recurs, I would not worry about this further.

Your girlfriend's various symptoms are non-specific and certainly do not suggest HIV.  On the other hand intermenstrual bleeding could suggest an STI or other abnormality and might warrant a check-up for her (more on this below).  That your penis was exposed to her blood does not change your risk for HIV.  There are no data that suggest that exposure to a partner's blood during unprotected intercourse change the risk for acquiring HIV, if exposed. 

Finally, your rash sounds like folliculitis and your upper respiratory symptoms are not suggestive on HIV or any STI.

Thus, in summary, little that you have described suggests HIV. My suggestion to you is that you and your GF might go together for a sexual health check-up including STI/HIV testing.  You have both had prior partners and have already done the hard by talking about past sexual partners.  By doing this you will each have confidence that the other one does not have an STI that might be transmitted to the other.  At the same time, this would also provide an opportunity for your GF to have her vaginal bleeding evaluated as well.

Hello dr, thank you for your response, her bleeding was due to us having sex and me ejaculating in her, thus, she took plan b and unexpectedly started bleeding while we had sex, since i wrote this post close to two weeks have passed and we have had unprotected sex over 30 times now maybe more, i have not developed any other symptoms other than the same folliculitis is still there and i sometimes developed some on my forehead and arms. The deenching night sweat episode was only one night, i have not had fever or amy typical symptoms, we spoke further about her previous relationship and it seems the guy was somewhat promiscous which highly worries me, she says shes not worried and she went to the gynecologist today.  What do you think are my odds of her having hiv and me contracting it, we will get tested soon thanks

Despite the fact that your GF's prior partner may have had many other partners, from what you have told me, there is little chance that she has HIV.  My advice is unchanged.  EWH---

Thank you Dr. for example since yesterday i have besn feeling a full bladder, but very lottle urine. Could it be anxiety or a uti again. My gf went to the gynecologist and he said she had a slight infection.

Either or anxiety a UTI could account for your symptoms.  The more likely cause is anxiety as UTIs are very uncommon in men under the age of 40.
