Dear Doctor,

A good day to you. I would like to seek your opinions about the experience I had, and I would be grateful if I can get some advices on if I need to test for HIV and STDs.

A day ago, a male, whom I met on-lion, came to my place and we had mutual-masturbation (I am male too). The scene was he lay on top of my body and used his penis to rub mine and areas of my perineum (I faced up too). I thought he did not insert his penis into my anus - we agreed before. Also, it was not easy for penetrative sex since I should tell if it happens. Finally, he ejaculated, and his sperm contacted my skin – some of which contact skins around anus.

I understand masturbation may not be a necessary exposure for test. Yet, since the sperm may contact anus where the skin may be different from elsewhere of the body. Maybe the skin of anus may have small cuts or graze (hemorrhoids?) that were not easily recognized or felt.

In view of the above exposure, do you think I need to test for hiv and stds? Can I have sex with my current partners (receiving oral sex by the partner?) I use condom for penetrative sex and I have fixed partner except for the abovementioned event once.

Welcome to our Forum.  I'll be glad to comment.  Mutual masturbation, even when partners get their partner's genital secretions on areas like the rectum or groin are no risk events in terms of HIV and risk for other bacterial STIs is miniscule.  In addition, I agree with you that if he had penetrated you, you would have known it.  As a result, I would classify this as a virtually no risk event for most STIs.  I see no need for testing related to the events that you describe.  Similarly, I see no reason related to this exposure to worry about the receipt of oral sex with your regular partner.

I hope this comment is helpful.  If any part of my reply is unclear please feel free to use your up to two follow-up questions to seek clarification.  Take care. EWH

thank you very much, Dr Hook! Your comments are comprehensive.  A minor question, that is to say, a graze ( that skin areas are thinner so this may happen?) around anus wound not make a different situation to worry? Many thanks!

Scrapes, abrasions or even cuts would not change my assessment or advice.  Without penetration this was stil a no risk event.  EWH
