[Question #3396] Testicle Pain (To Dr. HH)

Hello, i dont even know where to start, so i've had testicle pain for 8 months now, it started after i had unprotected sex with a girl at a party, i got it 1-2 months after the encounter, im 20 years old btw, i've been going to the doctors since it started, and they gave me antibiotics with no effect the first time, 2nd time no effects aswell, 3rd time it felt like it dissappeared for like 3-4 weeks, i had 3 ultrasound of the scrotum, but doctors said nothing out of the ordinary, its worth mentioning that i got a girlfriend right before the pain started, which im still with and scared to death i have given her something, anyways, i got no discharge, i haven't had any trouble with peeing, pain while peeing or anything, only once for a week a few months ago, but i dont think it was that, so i will try explain it in details now. my testicle isnt swollen, its not painful to touch, it feels rock hard though, and sometimes the pain is in my lower back and front, doctors says i cant go to them anymore with this problem cus they cant help me more. So what have i been tested for? I got tested for chlamydia and gonorrhea, HIV and syphilis, all came back negative, its also worth mentioning that i got tested for claymydia and gonorrhea three times, all good with that, im on anti-depression medicin now and stopped my college cus im so depressed and so worried for my girlfriend, i asked the doctors to test me for some other stuff, like NGU and HSV, but they refused since i dont have any other symptoms. After being in pain for 6 months, the pain started  to turn into constantly tingling/moving feeling in the scrotum, i told it to the doctors, and they said they never heard about that, im confused as hell, and im so afraid i never get my life back, im filled with anxiety and depression, i used to be fit and workout everyday, now i smoke a pack of cigarettes everyday and i lost 40 pounds. I also tried 4th antibiotics and avoiding sex with my GF, for 2 weeks. No effect either.

Welcome to our Forum.  I will be answering your questions today.  Dr. Handsfield and I share the Forum equally and have never disagreed on the advice we provide.

From what you say, it is most unlikely that your scrotal discomfort is due to an STI.  The symptoms you describe are not those of epididymitis.  Further, the STIs which account for nearly all testicular pain are either Gonorrhea or chlamydia and you have had multiple negative tests for these infections as well as probably taken antibiotics which would improve the infection if it were there.  Your treatment would have cured NGU and your symptoms are not compatible with herpes.

My advice is to see a urologist for this problem.  There are many non-STI syndromes which could cause your symptoms and a urologist would be most likely to be able to help.

Thanks for the reply Edward, im sorry for the late response, im worried about trichomoniasis, mycoplasma and HPV. I have seen the urologist 3 times, and they said they couldnt see anything on the Ultrasound, would it still be possibly for me to have pain due to a STD even tho the infection isnt seen on the Ultrasound? Since they say there is nothing to be seen they told me i just have to live with it, but its been 8 months of pain every single day, something i would like to mention aswell is like i said with the antibiotics, the 3rd time i took it, it dissappeared for some time and it the pain actually got into the right testicle instead of the left one. Something i also need to mention is i experienced once that after i pee there was coming cum out after i was done peeing, im so worried about those 3 STD's i mentioned above, and im so scared i could have past some over to my girlfriend, what is your advice to give me some peace of mind, should i just go and get tested for those 3 even though they told me they dont wanna do more test on me? i would just love to have some peace for once. Im sorry for my poor english, its not my native. And thanks so much for your help, it is very much appreciated.

Neither trichomoniasis nor HPV have ever caused testicular problems/epididymitis of the sort you describe.  There are rare reports of mycoplasma genitalium causing epididymitis but your symptoms are really not suggestive.  Urine testing is available for M. genitalium and if you choose to test for this and are negative, that would rule out this most unlikely possibility. 

My sense is that the effect that antibiotics had for you were more coincidental than suggestive of an STI response. 

I would try a different urologist for a send opinion.  This really does not sound, in any way like an STI and certainly is not due to HPV or trich.  EWH

Thank you so much Edward, felt amazing to hear your opinion, im so happy i asked in here, and yes, i will try see another urologist, and again thanks so much, what a relief you gave me, probably still gonna take mycoplasma test, but i appreciate your help really, and i think this would be all, keep up the amazing work, you help a lot of people.
