[Question #3464] Worried

Hello doctors,
I am a 35yr old male and approximately 5 months ago I received brief oral sex at an Asian massage parlor. Roughly 7 days after this even I began feeling a slight burning after urination, lower abdominal pain, and a general discomfort in my left testicle. There was no discharge of any kind.
9 days after the event I went to a walk in general clinic. I was told I needed to take a urine test. I had urinated roughly 30-40 min prior as I was not aware that I would be taking such a test nor did the RN mention the requirement that I not have urinated for at least one hour before. They performed NAAT tests for G, C, and T. All tests were negative.
Following this test the symptoms appeared to fade and all that was left was mild testicle discomfort and lower abdominal pain. Roughly 1 month after the event I was in the hospital for gastrointestinal issues and treated with IV metrodinazole and ceftriaxone for 3 days. Since then I have been on 2 separate 14 day courses of metrodinazole and cipro.
I have discussed these symptoms with my gastroenterologist and he feels they are normal for my condition, especially the burning and painful urination, and the testicle pain is probably referring from the intestines.
What worries me is that the pain in my left testicle continues. To clarify, the testicle does not appear swollen nor is it painful to the touch it’s just general pain and sometimes I’m not confident it is the testicle that is hurting.
My questions are: was my first urine test a reliable test? Would you recommend additional testing? If so, what would the test show given the amount of antibiotics I have taken? Does this sound like epididymitis? Could this really be driven by anxiety? Is it safe to resume normal sex with my SO (about a month ago she had a clean test as part of a visit with a new OBGYN)?

Welcome to our Forum. I'll try to help. I can understand how the timing of your symptoms onset led to concerns about the possibility of having acquired an STI and had you not been tested I would have suggested it. Gonorrhea, chlamydia (very rarely) and non-chlamydial NGU can all be acquired from receipt of oral sex. Your testing however provides reliable proof that you do not have gonorrhea, chlamydia or trichamonas and the fact that the testicular discomfort persists after both 3 days of ceftriaxone and several courses of ciprofloxacin further indicate that this is not an STI acquired from the exposure you describe.
Testicular discomfort of the sort you describe can have many causes including not only STIs but also can originate from gastrointestinal sources, the kidneys, and other intra-abdominal problems. In some persons mild discomfort can persist for long periods without an obvious reason. You have done the important tests for STIs and can be confident that this is not an STI. In answer to your specific questions:
1. Yes your test results are reliable There is no need for additional STI testing.
2. As I said earlier, epididymitis might have been a concern early but has now been ruled out.
3. Anxiety can accentuate discomfort of this sort and may be the origin of your current symptoms.
4. There is no reason to hold back from unprotected sex with your so.
If the problems do persist, my advice would be to check with a urologist to be completely sure this is not prostatitis (non-STI) although your treatment thus far would have taken care of this. I would not worry about an STI. I hope these comments are helpful. EWH

Thank you Doctor Hook. I have a few follow up questions so I can better understand your response:
The initial test was reliable even though it was not done under optimal testing?
Even if it was not reliable, the antibiotics I have taken would have cleared any STI?
If this is epididymitis, and you are confident it is not, the antibiotics I have taken would have cleared it?
I apologize for the follow ups but the coincidental timing of everything has really been eating me up for the last few months.

Yes, your initial test was reliable. Recommendations that tests be performed after at least one hour following last urination are overly conservative. Believe your test results. Furthermore, the antibiotics you took would have cured any STI present if you were infected.

Thank you Dr. Hook. Your comments have been a tremendous relief. Again, the coincidental timing of symptoms coupled with anxiety and the fact that the testicular pain and urinary symptoms come and go and don’t always coincide with my gastrointestinal discomfort had me very worried. Thank you again.

I'm please my comments were helpful. I would just add that when we are worried or anxious and, as a result, are on the lookout for signs of infection, it also increases the likelihood the we will take note of coincidental occurrences. Have confidence in your test results. Take care. EWH---
