[Question #3486] HIV Risk Assessment please

Hi Doctors, I'm male 33 year old and live in Jakarta, Indonesia, so pardon my English.

First of all, I am a sexual active and had last negative HIV test in 2016. From the last test until early January 2018 I had approx 7-10 sex encounters with sex workers in massage and spa parlours, all vaginal sex was protected with condom, once I had unprotected oral sex, and twice I had deep kissing (french kiss). I never had fear about HIV because I almost 100% believe that the condoms were not broken (not sure if there was a pin hole or something similar like that), the condoms always intact on my penis, and I also always ejaculated inside the vagina and the semen was always stay inside the tip of the condoms.

My fear and anxiety suddenly changed after the last encounter on the first week of January 2018 (forgot the date), I had protected vaginal sex and deep kissing. One day after, I got cancer sore, bleeding gum, and 1-2 weeks after I got dry lips and small red bumps on both arms (they are come and go about 5-10 bumps). Googling of symptoms drove me to get HSV 1 & 2 test (At that time I concerned about herpes, not HIV) and the result came HSV 1 igg positive and the rest are negative. I have had a consultation with dermatologist (Here in Indonesia is skin and genetically specialist) and the doctor said HSV1 igg is an oral herpes and is not a big issue because I never had an outbreak. And the doctor said my dry lips was due to tooth paste alergic.

Mid of February, I suddenly got dry mouth, white tongue, still dry lips and dry eyes. And again, searching on google made me extremely concern about HIV. All realated to OHL and sjrogen's syndrome.  I was really stress and scare to get HIV test. My fear and anxiety are crazy. I also have wife and 2 yo son,  and I scare have infected to my wife because we had unprotected sex since after the last encounters.  Do you think I need get HIV test?Do you think all my symptoms are related to ARS?Can I continue have sex with my wife?

Welcome to our Forum.  Your English is fine.  I will be glad to comment and hope that my comments will help to relieve your concerns about HIV.  It is unlikely that any of your sex partners had HIV- most commercial sex workers do not have HIV.  In addition, your sexual activities are all in the category of safe sex.  Condom protected genital sex is safe sex as long as the condom is worn throughout the encounter and does not break and there is no reason to think that your condoms may have broken- when condoms break, they break wide open leaving no doubt that they failed.  The other parts of your encounters, open mouth kissing and receipt of oral sex are also safe and have never been proven to lead to acquisition of HIV.  Thus none of the interactions you have had with the CSWs suggest risk for HIV. 

Your symptoms do not sound like the symptoms of HIV either.  There are many causes of oral sores, dry lips, bleeding gums and HIV are rarely, if ever part of the appearance of recently acquired HIV.  Similarly, a white tongue is typically not due to thrush or other HIV-related processes.  There occurrence is co-incidental and unrelated to your sexual activities. 

Your doctor is also correct about your HSV-1.  Over 60% of adults have HSV-1 and most of them have not had cold sores.  I would not worry about this test either.

In summary, I see little reason for concern about HIV from what you have told me and therefore there is little reason for you to worry that you could have infected your wife.  Given your anxiety and concern, my advice is to get an HIV test- the results should be conclusive at this time and will relieve your anxiety.  Testing will not change the result, only prove (in your case) that  you are not infected and make you feel better.
