[Question #3494] Itching, redness, tingling crawling and no blisters.

I have been itching for the last two months after a protected encounter with a female. itching started in four days and I have seen dermatologists and a GP none of them say this is Herpes related however the itching does not go away. They did not see lesions however. My groin and anal areas are very tender (reddish and can feel the pubic hair rubbing my skin but no one saw bumps, I felt on or two cuts in the anal area as they tend to cause stinging sensation while showering) and have been itching or feels slightly burning once in a while and constantly feeling like something is crawling in that areas. Visited three dermatologists and one GP. All advised it is not herpes and two of them gave steroid cream and two dermatologists prescribed antifungal. I don’t know which of them are right? I am driving myself crazy reading too many forums. Is this some sort of asymptotic herpes where the symptom just shows as itching and redness? My first two weeks of itching was intense and eventually subsided while applying the steroid cream but it return when I stopped applying that cream for 2-4 days. applying antifungal or steroid cream it gets better but the redness returns in few days again if I stop applying. Could it be herpes? The doctors days they don’t see blisters so they don’t think it is herpes. My 4 week testing hsv1/2 is negative. I m safe or do I have to wait till 3 months? I read from Terri’s post that IgG won’t pick up hsv2 8% of the times,May I know why? Do you think the lesions might show up later time or it would have shown right away? It has been 6.5 weeks since exposure almost. I read forums in which people found out having An out break after 6-8 months? Should I trust the result after 3-4 months and Should I trust my doctors?

The issue with test not picking up 8% of HSV 2 infections compared to the herpes western blot is a lack of sensitivity of the test, not something about the person. This does not seem consistent with herpes to me because it would be unusual for new herpes to present with only itching and not any lesions. Also, steroids would make herpes worse, not better, as it takes away the cellular immune response in the skin and make lesions more difficult to control.
Also, the fact that a condom was used greatly reduces the risk of herpes being transmitted to you. Do you know if this person had herpes or not?

Hello Terri,
This is a one time thing and she is a prostitute. I used anti itch cream from cvs during the first few days after the infection and it also seem to help until the time I had it on and the itching returned after shower, then I started approaching dermatologists thinking that it is getting more complicated in the end of the first week or 10th day roughly! Can a herpes out break get helps subsided with a cvs anti itch cream?
Also I have been having this crawling/itching feeling all over my mouth and lips since morning in addition to the same feeling downstairs.
I have been feeling like may be I passed on Something on to my face from my down side (genital to oral herpes may be)? Do you think if I have herpes I will have these sensations on lips and itching intensely even after 6.5 weeks after possible exposure? This crawling and itching on above & below lips seems to be coming up more from this morning, however the itching seems to be more persistent down in the groin area. I don’t remember having body temperature beyond 99 F during any days in the last 6 weeks nor sore throat discomfortness beyond 2-3 days constantly. Can herpes show without those fever and flu and enlarged lymph symptoms?
What is an asymptotic infection in herpes?
Can it attack us and show without any or few symptoms like what I have been experiencing?
Should I relate this to herpes or should I just trust the result and move on with whatever my doctors said? My dermatologist seems to be in practice for 50 years in Indianapolis area and I know he probably would have seen many cases but this itching is making me feel more worse. Can I trust my doctors?
I don’t know why am I having the itching feeling/sensation? So just having rash is not a symptom of herpes and not having lesions? I have read in multiple websites that people caught it and never had symptoms and BAM! a blister/sore shows out of no blue, why would that happen? Is herpes hard to diagnose? In one of the forums one guy said he got the herpes blister after negative testing after 8 months of exposure, such thinks make me so much worried. Please help!
Should I keep using the steroid or fungal cream? Whom should I trust? Does it sound like jock itch or just eczema or try valtrax? My doc not knowing what to do she said she will write valtrax if I wanted to but I refused to take it without proper diagnosis... what would be your suggestion based on what you have seen?

One thing I have noticed is that the itching is persistent in the areas where skin is tightly folded or having less air exposure such as area between rectal and the area transitioning between rectal and scrotum.
The area between thigh and groin does not itch much it has significantly subsided but not the bottom area where there is constant chaffing of underwear and thighs.. I did not notice any itching on penis can herpes be more localized? If so why would my rectal areas itch? My rectal area never came in contact with her skin, she was on top for 30sec briefly and that is when her skin touched my thighs and groin area for a significantly brief time. Area above penis or pelvic area did not itch, it is the closed areas that have been itching badly.
I carefully removed the condom and cleaned hands and never touched it and touched my Genital area. This is what happened and I ended up itching in the next few days. No exposure to bodily fluids also..

What does the dermatologist think is going on here? This presentation is just not consistent with herpes.
have you tried an antifungal medicine for the itching? You mention using an anti-itch medicine but what was it an antifungal.
It is simply not consistent with herpes for the only symptom to be itching and for it to continue to itch for two months - not consistent with herpes at all.
This is an issue that needs to be resolved with an in-person clinician through swab testing of the area for bacteria and fungi. This may or not have anything to do with the encounter that you had with the sex worker.
