I got a full-panel check of STDs one year ago including HSV-2 antibody and the results were all negative. Since that, I had unprotected sex with my ex-girlfriend(last time was 4 months ago). I visited CSWs 3 times in total and I have unprotected oral sex with one of them for 2 minutes 4 MONTHS AGO. All other sex were condom-protected and I checked the condom every time after the sex. Last time I visited the CSW was exactly 1 month ago in U.S.

Since I clean and examine my penis everyday and I think no symptoms showed until ONE WEEK ago:

I met another girl 2 months ago who said she never had unprotected virginal sex with others before and she also got checked. We had unprotected oral and virginal sex few times. One week ago, I saw some red rash on my glans with itching which disappeared in 3 days while I saw some white dry secretion from my penis skin instead of from the urethral opening. I thought everything went well but then I saw an ulcer on my frenulum of prepuce. (I always have a CUT on it after sex). Also a small tiny red blister on the coronary sulcus. At that time I felt no pain in urination. I went to the hospital and the physician said 75% it is fungi infection and 25% it is herpes. I am still waiting for the Herpes test result. I applied the anti-fungi cream for 3 days and the symptoms were not improved. Another 3 blisters on coronary sulcus appeared today and the ulcer is still here, also I am having swollen lymph nodes and painful urination, which make me believe that I have the genital herpes.

So if it is the herpes, I think it is the first time outbreak according to my understanding and memory(95% sure). Do you think this girl past the virus to me? Or can I catch the virus from other episodes (protected oral and virginal sex 1 month ago or others sex much longer time ago?) What is the possibility?

I will let her check herpes for sure but please give me some ‘educated guess’ on how I could be contacted. Thanks!!!

It is very important that you present to your clinician to have these newest blisters swab tested AND typed (type 1 or type 2) if indeed the swab is positive for herpes.
If the swab test is positive, you will know that you contracted it sometime in the past year. Condoms are useful in preventing transmission from females to males but they are not perfect.  If the swab test is positive for HSV 2, you will know almost certainly that you contracted this from intercourse.  If it is positive for HSV 1, it is likely from oral sex.  These are not 100% true statement but mostly true!  Please got back in today and get swabbed again and if possible, ask for a NAAT or PCR swab test vs. the old style culture.

Hi,

Yes, this could most certainly be a false negative.  The IgG test for HSV 1 misses 30% of infections compared to the gold standard western blot.  She could obtain the herpes western blot to verify her HSV 1 status.  It is also possible that you could have had this for a while since your presentation with a first blister was very mild.  If your partner does do the western blot and is negative, then this is something that you've had prior to your encounter with her.
