[Question #3703] Oral sex for 2 seconds from a stripper concerned I may have an STD.

I went to a strip club 13 days ago. I was asked to go in the back for a dace so I did. I was danced on for a 2 songs. The stripper would put her hand down my pants and give hand job for 2-3 seconds and then would mostly give a lap dance. Durning the second song she continued to give a hand job 2-3 seconds a time but then she pull my penis out and suck the head only, for 2 seconds before I stopped her. She then turned around and was dancing on me pulled my penis out again and rubbed it against her underwear and was pushing it up in. The penis never went inside her through the underwear but I could feel the tip rubbing between her labia through her underwear this lasted 2 seconds before I stopped her. Afterwards I went home and concerned about STD's i decided to wash my genitals with Dial hand soup and water. I notice the glans was irritated and it felt irritated, there was some small tiny red dots around the rim. A couple days went by and my glans of my penis was hurting along with the tip of my penis. I decided to get tested for gonorrhea and chlamydia. I went to an urgent care i did a urine GC and chlamydia along with a dip stick at 7 days after the night of the strip club. The Dip stick showed no leukocytes/nitrates and the chlamydia/gonorrhea test was negative. The urine culture has not come back yet. Im still having a burning sensation around the rim of my glans and at times tip of penis. So I started to apply 1% hydrocortisone which seems to help. I noticed that my underwear was really irritating me while at work so for the past 2 days i've been covering my penis with gauze and it seems to improve the symptoms dramatically but there is still the sensation of burning at the rim and sometimes at the tip. There is no burning with urination of drainage at the urethra. I continue to look at there still is some red dots/blemishes on the rim of the glans on the left side that appear to be constant since the night. Around the Urethra appears irritated still and especially after a day at work walking around. I have not seen any blisters or fluid filled vesicles that burst and left a scab.
My question is
1. What are the chances I contracted Herpes, syphilis, HIV or hepatitis?
2. Does my symptoms sound like herpes. When do herpes outbreak occur after the initial encounter?
3. Can you have herpes without the painful vesicles?
4. Should I still be this concerned. And when should I feel safe to continue sex with my girlfriend.

Welcome to the Forum and thanks for your question. the exposures that you describe were very, very low risk for any STI and the course of your symptoms is most consistent from irritation (friction) which occurred during your receipt of masturbation (the hand job). The irritation you noted occurred soon after your encounter, something that is typical for friction related irritation. Irritation from STIs typically takes several days to occur.
1. What are the chances I contracted Herpes, syphilis, HIV or hepatitis?
Virtually zero. No one has ever gotten HIV or hepatitis from receipt of oral sex. Herpes is rare and typically symptoms of herpes would have most likely appeared by now. The irritation that you describe does not sound like herpes. Syphilis is even rarer than herpes following oral sex. I would not worry.
2. Does my symptoms sound like herpes. When do herpes outbreak occur after the initial encounter?
No the irritation you describe does not sound like herpes. Typically, the lesions of herpes appear 7-14 days after exposure. You are now 10+ days since your exposure making it unlikely that you got herpes.
3. Can you have herpes without the painful vesicles?
Yes, but as I said your risk if very, very low.
4. Should I still be this concerned. And when should I feel safe to continue sex with my girlfriend.
My advice is to not worry. Nothing you have described suggests any need to abstain from unprotected sex with your GF.

Hello Dr. Hook,
Thank you for your timely feedback and you Medical advise. I have been continuing to have issues with pain to the glans area still no pain with urination and quite significant pain with boxers rubbing against the glans. I found a spot on the sole of my foot that seems to be painful it is non-blanching, could this single spot be secondary syphilis and can syphilis rash only be 1 lesion ? Also would the secondary rash occur without a noted genital lesion and is this the normal time frame for secondary syphilis ? I do remember walking up steps barefooted and feeling the head of a nail press up into my foot but I had no other symptoms after and no bleeding which may be the reason for the lesion. The glans rash is still painful and blanchable with palpation almost like a sunburn.
If this is not herpes or syphilis do you have any recommendation on treatment for this rash/ pain to the glans.
Also Do you allow photos to be uploaded for you to examine ?

My sense is that related to the events you described you are now examining yourself far more closely than you ever have in the past. Many people who do not have syphilis have one or a few lesions on the soles of their feet or on their hands. It is too early for a secondary syphilis rash to appear on the sole of your feet and if you had a syphilis rash, it would not be a single lesion.
The irritation you are experiencing on your rash still sounds nothing like an STI. My advice is to stop examining yourself (particularly the glans of your penis (you may be making it worse!), to wear soft underwear and not worry about it. Application of 1% hydrocortisone cream twice a day for a few days may also make it feel better
Sorry we do not look at phots on this site. Past experience has shown that it is not helpful and is often mis-leading.
I urge you not to worry. There really is nothing you have said, either about your encounter or about your perceived signs and symptoms which raise concern that you acquired an STI from it. EWH

Thank you for the advise symptoms seem to persist yet and fluctuate in intensity. Depending on the day and possible irritation. I do have 1 Final question which I should have asked earlier. In your experience was the urine gonorrhea/chlamydia test 1 week after the possible enough time to get reliable results and should i trust the negative results. I also received the urine culture there was no growth seen.
As this will be my last question I just want to thank you for your help in this manor. The work you do is truly appreciated by many including me!!

Currently available urine diagnostic tests are more sensitive for detection of gonorrhea and chlamydia than any tests ever available. They are highly reliable and should be believed. Test results taken any time more than 3-4 days after exposure are accurate so your 1 week results are reliable. I urge you to believe these results, stop repeatedly examining yourself and move forward with confidence that you do not have STIs.
