[Question #3835] CSW/Oraquick

Im a male in my late 30s  Five weeks ago I had unprotected oral sex (I was the receiver) and very brief unprotected sex (5 minutes would be an exaggeration) with a female partner, an amateur CSW from one of those Sugar Daddy sites. She's in her mid20s. I asked her after the event if she had any diseases or did drugs, she said no to either but I don't quite trust her since her other profile on the other dating site says she doesn't give a *@!$* about anything. 

  This is in New York. (Long Island)

A week later I take an Oraquick oral at home test. I know too soon to be any use for this encounter. But it was negative. 

Today I took a other one. Again negative. 

Should I keep worrying? More tests?

Welcome to the Forum.  I'll be glad to comment.  The odds that you got HIV from the exposure you describe are extraordinarily low.  The chance of having HIV, even as a CSW is less than 1% and even if she was, the risk of getting HIV from a single encounter is less than 1 in 1000-2000.  Thus your mathematical risk of infection is substantially less than 1 in 100,000 or about ten times lower than your risk of being struck by lightening at some point in your life.   OTOH, your risk of getting one of the far more common STIs, like chlamydia, which is more than 30 times more common than HIV, makes testing for other STIs worthwhile. 

You are correct, the Oraquick at one week is not worth much at all.  At five weeks it would detect 80-90% of recent infections but amongst HIV tests, the Oraquick is our least favorite and results would not be entirely conclusive until 10-12 weeks after exposure.  My advice is to go to a clinic and get lab-based tests for gonorrhea and chlamydia and a 4th generation, combination HIV antigen/antibody test.  At this time both results would be conclusive and you could move on with complete confidence and without a need for further testing.

I will and saving my last question to post the results. 

Two follow up Q. I have been having a sore throat for the last three days, but no fever or rash to speak of. The sore throat is what made me.go out and get the home test today.  If it was ARS would the test be positive ?   

Second q - any thoughts on the online STD tests, in particular the one that you pay then go to a lab and get the tests done. StdCheck

Your sore throat is not due to HIV.  First, isolated sore throat without other symptoms (such as high fever, severe muscle or joint aches, etc.) does sot suggest the ARS or recently acquired HIV.  Second, 5+ weeks is a bit too far out from the exposure to suggest the ARS.  Finally however, typically, if your symptoms were due to the ARS, the OraQuick most likely would have been positive.

The quality of the tests used by most of the on-line testing services is typically high, particularly if they are working with respected licensed labs.  The counseling offered with such tests however is spotty.  Often just as easy, and cheaper, to seek confidential testing at a local health department.   EWH

And I hate to be that guy. But why is there such a wild range of what's conclusive?  Have you ever seen a 5 week negative with a 4th gen test become postive later (without another risk event)?

Neither of us on this Forum have ever seen someone with a 4 week negative a combination HIV antigen/antibody test become positive afterwards but apparently, according to our CDC colleagues, it does happen albeit quite rarely.  The broad range in results reflects various degrees of conservatism and, in some (particularly on the internet) instances, misinformation or information that is out of date.

Thanks for sharing your results.  Consider yourself in the clear .  I encourage you to go forward from here with no further concerns.
