[Question #3894] Chances I could have HSV and symptoms

I had encounter with a prostitute 9 weeks ago. Not an expensive csw but one off the streets (very stupid). 

Since, I've been extremely worried about all std, especially hsv. Here's some info and can gladly give more. Was one time and a condom was used. Involved oral for about a minute followed by doggy for about 2 minutes. Afterward, she told me she had hsv and may have had an ob as she had itchy crotch. I'm worried she may have had sores cuz she insisted the lights be off. 

I've checked myself constantly and never saw any blisters or fluid filled sores. I did experience tingly, itchiness, and hot/ sweaty genitals starting the first week after. Had a dr visit at 27 days and he said all looked well. I also tested for all std except herpes (he wouldn't do it even though I asked) and all were neg. I have recently noticed an issue under my shaft. It appears the skin is bunched up and there is an appearance of vertical lines or cracks. When I spread them apart with my hands it looks as if there is a round spot like top layer of skin is gone and  if a blister was there and healed. There is no fluid, scab or any pain when I touch and spread the skin but when I'm walking, I definitely feel pins and stings every so often. There are about 4-6 of these spots and almost all are right at the middle of my penis on the underside of my shaft, along the thick line of skin that runs from the tip and separates the scrotum (idk the proper term). 

My main questions are could this issue under my shaft be herpes? What are the chances I'd contract herpes from this one encounter even though it was high risk?

Just wanted to add some additional info. Prior to this encounter I've only been with my wife. We have been together for 13 years and she was clear and tested for things while pregnant I believe. Not sure if they test for herpes in pregnant women. So if I have herpes I'd be able to definitely say it came from this one encounter. Seeing as this person told me she had unprotected sex with hsv pos men, shared needles with hep c infected people and didn't care to be tested, I decided to get Tested for all std at 27 days and 9 weeks. All were negative. 

After seeing these spots under my shaft, I freaked out and got an IGG test from quest. I just got the results and was negative for both. I didn't get any values though so I can't see how close they were to being positive. They told me I would only get exact values if I tested positive. Since test was done at 9 weeks post exposure, how accurate can it be? Should I consider the western blot just for piece of mind and since I have some issues under the shaft? I still have my appointments next week at planned parenthood and dermatologist to get their opinions on the spots.

At 9 weeks,I would say there is probably a 90% chance that you will continue to be negative, if you tested at 12 weeks.  The condom was your friend.  I'm not sure what the things are on your penis that you describe but I'm not thinking from your description that this was herpes.  I think herpes lesions would be more noticeable than what you describe.  If you want to do the blot at 12 weeks, I don't see any reason not to. Peace of mind is priceless.  Do I think you need it?  Not really but we both might feel better if you had one.  Let me know what the derm says.
Given these circumstances, my guess is this has been incredibly stressful for you.

Hi Terri,

Thank you for writing back. Yes it's been a nightmare to say the least. It's consumed me and all I seem to worry about 24/7. Initially, I was worried about hiv then shifted to herpes. It was a stupid decision and now I'm doing all I can to try and move past it, just want to verify I'm clean so I don't pass anything to my wife. I have read many of your answers on here and I read the handbook on your westoverheights website so I feel much more informed now. 

I know it's tough to describe these spots so I will definitely let you know what the derm and clinician at plannedparenthood say. In the meantime, I have a few things I was wondering if you could give your thoughts on...

1.  After reading a ton of info, it seems my chances of contracting herpes after a one time, condom protected encounter are slim. Would you still say that the chances are slim even though this individual was hsv2 pos and of higher risk due to drug habits and not caring about infecting others since it's how she makes her money? 

2. Since I have a very limited sexual history (only my wife prior to this) and tested negative for both types, would I most likely have had a noticeable ob by now if I was infected? Especially if she had an ob during the encounter? That's one of my main worries because she insisted it be dark so I never saw her genital area so after she said she was positive, all I seem to think is she had to have had an ob, which is why she wanted it dark. 

3. I've had sex with my wife several times since the encounter. If I was positive, is there a good chance she would have been symptomatic by now? She hasn't said anything which makes me feel better. Would you think it's safe to continue having sex with my wife at this point?  

4. These vertical lines seem to show a break in the skin when spread apart but no pain, fluids, etc. Is it possible to have herpes and not have fluid filled sores and just a break in the skin, or are they usually painful and some fluid?

5. If I get an IGG done again  at 12 weeks, would that be conclusive, would it be any more accurate if I waited closer to 16 weeks? 

6. Know it's hard to say for sure but with it being only one encounter, no obvious symptoms, condom being used and a neg test at 9 weeks, overall do you think my chances are pretty good that I'm negative?  I have the burning and itching occasionally but not sure if that's something else as I heard those can be symptoms but often come with sores and would not last for weeks and weeks. 

I will most likely reach out to you for a wb as well. My doc will not do it, he didn't want to test at all for herpes. I have found a lab here (any lab test now) that said they will draw and send off the blood as long as I take the shipping labels.

Oh yes, incredibly slim!

Yes, I believe you would have more remarkable symptoms for sure!

Given everything you have told me, yes, I think it is OK but I never say never.  I just can't ever be 100% certain about this.

This does not sound like how herpes behaves to me.

12 weeks is very good.  No need to wait 16 weeks but the IgG test is not perfect compared to the western blot - very good for HSV 2 but not as good as the western blot

I think the chances are just very very good indeed that you are not infected!
