[Question #4020] Penis tip tingling, inflammation, dribbling sensation

Hi Doctors,

Just wanted to say thanks for what you’re doing on this site. I’ve reviewed many of your prior posts and have found them to be very helpful.

My situation:

-          May 12th: Unprotected sex (condom broke, unsure how long) with woman who was in a relationship

o   This is the second person I’ve had unprotected sex with, 3 weeks after she confirmed she feels completed normal

-          May 14th: Two days after unprotected sex, I began having a strong tingling and leaking sensation at the tip of my penis constantly throughout the day, however, whenever I would check there would be nothing there. Redness and inflammation started a week or two later. No other symptoms, as I checked vigorously over the last few months

-          May 14th: went into doctor, STD tested administered for HIV, HSV1/2, G+C, Syphilis, all negative

-          May 16th: STD tests come back negative, treated anyway with Z-Pack (1 dose) and ceftriaxone shot

-          May 21st: no improvement, go to doctor and tested for Mycoplasma hominis & Ureaplasma, negative. treated with Ciprofloxacin (7 day course), metronidazole (1 dose), and doxycycline (7 day dose)

-          May 31st: still no improvement, see a urologist who prescribes Uribel and says give it some time (uribel seems to have made it worse?), as he thinks I show no signs of further infection except for a little bit of blood in urine

-          June 7th, urine analysis still considered normal, tested for HIV, HSV ½, G+C, Syphillis, Myco Hominis / Genitalium, Ureaplasmas, all negative

-          June 19th: stop taking uribel

-          June 26th: retested for everything including HIV, HSV ½ (igG), G+C, Syphillis, Myco Hominis / Genitalium, Ureaplasmas, again all negative

-          July 18th: retested for HSV 1 and 2 IgG (10 weeks after exposure), still negative.

I am now at a spot where my symptoms feel as though they may not be as intense as they originally were, and are possibly slowly getting better. However, I still have some redness / inflammation at the tip of my urethra, as well as a sensation that urine is leaking out of my penis. I’ve avoided sexual contact during this period.

With this in mind, the way I see it is:

1)      I had NGU, and still have some sort of resistant strain that wasn’t showing up on testing due to the antibiotics

2)      I have persistent inflammation without infection, and just need to give it time

3)      I have Herpes 1 or 2, and this is an atypical showing of the virus (I have had no blisters, sores, etc. out of the ordinary)

Please let me know your thoughts on what you think might be causing this!

Welcome to our Forum.  I'll be glad to comment.  The exposure you describe was low risk (most people do not have STIs and most exposures to infected partners do not result in infection), you have been broadly tested on two occasions which were negative and you have taken antibiotics which would cure nearly all bacterial STIs (which, given your negative test results, you are unlikely to have)

In answer to your specific questions:
  Sometimes following an extramarital dalliance, some persons begin to examine themselves repeatedly and far more closely than they would have otherwise.  This in turn leads to observation of findings (normal discharge for instance) that went unnoticed in the past.  In addition anxiety can lead to urinary frequency which is NOT a symptom of any STI.

In a In a small proportion of persons with NGU symptoms can persist despite appropriate therapy.  In these situations, if tests are negative, partners are not at risk and the symptoms tend to diminish over time (sometimes taking weeks or longer).  Such symptoms can also be heightened by heightened, excessive manipulation during the course of repeated examinations.  I suggest you give it time 

3)      I have Herpes 1 or 2, and this is an atypical showing of the virus (I have had no blisters, sores, etc. out of the ordinary)

ThTThere is no reason for you to think you have herpes other than paranoia.  While HSV tests are not perfect, acquisition of HSV from a single exposure is uncommon, you have not had an outbreak, your symptoms are not suggestive, and your tests are negative. 

My advice is to not worry, move on and do your best to not perseverate on the symptoms you have.  Nothing you have described suggests that you have an STI.  I hope this information and comments is helpful.  EWH

Thanks for your quick response. It was along the lines of what I was expecting. Overall, it is tough for me to chalk it up to “heightened awareness” given the physical inflammation and constant sensation that urine is leaking out of my penis / tingling, as well as “cold” feeling of the penis tip that I have never felt before. It just seems odd that this would start occurring 2/3 days after the unprotected sexual exposure.

 

To your comment about persisting NGU symptoms, this definitely makes sense. I have made a point to stop with the religious inspections a few weeks ago, hoping that it will bring the inflammation down. Also, I went in today to get one more round of tests for likely causations(MC, UP), using a first catch sample first thing in the morning, which is something I have not done for other tests.

 

In terms of HSV concerns, the only reason this is still in play is that I have read (through online forums, I know are not always trustworthy), cases of people who have had similar urethral tip discomfort (i.e. constant dribbling sensation, inflammation of the urethral opening, etc.) that have later found it to be HSV causing the infection. These were noted with no signs of outbreaks, even with inspection.

 

With the above concerns in mind, is there anything else that you recommend I can do to get to the bottom of what might be causing this, or at least put my mind to rest? Would you recommend continuing to test for herpes at 12 or 16 weeks to be conclusive? Or maybe trying antiviral medication for some time?

 

Please let me know your thoughts based on the specifics above!

I think you and I are going to have to agree to disagree on the issue of heightened awareness.  My thoughts on this were offered based on the nature of the symptoms you described and your numerous negative tests.  "Tingling" or "itching" are the sensations most typically experienced among persons who detect normal sensations in the course of looking hard for symptoms. 

I anticipate that your repeat tests will, once again be negative.  I note that you may have tested for Ureaplasma.  I would not have recommended this.  Current data are that both Ureaplasma and mycoplasma hominis are normal bacteria often found in the genital tract which are not associated with disease.

The symptom pattern that you describe is not suggestive of HSV.  Further, the persons who experience the sensations you describe typically have positive blood tests or experience such symptoms at some times but have experienced outbreaks as well.  Neither is the case for you.  Herpes tests are now considered to be conclusive at about 12 weeks.

Hi Dr. Hook,

Your response makes sense. I have come to terms with the fact that I likely do not have anything going on, and the sensation will begin to disappear with time. For my own sanity, I'll probably retest in another few weeks just to completely rule HSV 1/2 out and not need to feel worried about being with any partners.

I am still waiting on the results for the Mycoplasma (which I assume will be negative) but I did want to get your thoughts on one thing. I have been noticing these two strange white spots on the head of my penis (tried to upload a photo, but it wouldn't let me) with a small red dot in the middle. I started noticing it about a month ago, but they only appear when I have been sitting on the toilet for 10+ minutes or randomly sometimes after a very hot shower. Then, they disappear after standing for a minute or getting out of the hot shower. It's not every time, so it definitely comes and goes in those instances.

I never notice it unless I am in one of the above scenarios (sitting on a toilet, presumably b/c circulation is cut off, or after a very hot shower) and it has no texture that is different than the normal skin, no itching, no burning or anything else.

Do you have any thoughts on what this could be? This has been a leading cause of my HSV concerns, and that it is a weird type of outbreak.

From your description I would guess that what you are observing are dilated blood vessels.  The head of the penis is highly vascular and blood vessels can dilate with both straining on the toilet or the warmth of a hot shower.  The fact that they resolve so quickly is entirely inconsistent with herpes.
