[Question #4064] Hsv symptoms and questions. Muscle aches no lesions day 18

27yr old male, July 15 I received unprotected oral sex from a sex work followed by protected sex. During the oral she pressed and put her finger into my anus.
Day 1 after exposure pain while I urinated only day. looked up all the symptoms of everything online and freaked Day 2 a tingling sensation at tip of penis until day 5 it migrated to my anus. Day 7 it came back to my penis. Between day 7 and 11 I felt a migrating muscle pain from my left leg to my right leg down to my feet. On day 8 felt an extreme unpleasant general itchy feeling of my groin. Not one spot in particular. On day 10 I noticed 4 or 5 small red bumps form under the head of my penis where the forskin would be. Spaced out by a quarter inch. Never hurt oozed puss scabbed. It's also not where the tingling sensation was at. Day 10 an unconfirmed fever and chills and on day 11 also but only at night. Day 11 I saw a dr who performed a panel of test that came back neg including the swab test of the bumps. During this time I experinced high anxiety, tightness in chest, sore throat lump throat choking feeling loss of appetite leg pain. It would appear that as soon as I read a symptom I experinced them except for the migrating leg pain. I cannot remember at all in which order that happened. The doctor prescribed lotrim af for 7-10 day he thought it was a yeast infection. Red bumps disappeared in less than 3 days and the tingling went away in 1 day. Saw another dr on day 15 for ARS symptoms. Probably anxiety as the doctor said.

Since day 10 (now day 18) I've had a mucus feeling sore throat and leg weakness/constant ache on my right upper frontal thigh. Dry mouth and bad taste, I've noticed that the tip of my penis is irritated with a red blotchiness color and is exactly where its was over sensitive. Does not respond to the lotirmin af

1- do initial outbreaks cause flu symtoms and tingling? I've only read these symtoms which are prodrome meaning after the initial outbreak? Any happening prior to first outbreak?
2- how long after the prodrome symtoms do the outbreaks happen?
3- can anxeity cause any or all of these symptoms? Especially the leg muscles?
4- do all hsv lesions blister and ooze?
5- what risk for HIV and HSV?
6- when and what type of test is recommended for HIV/HSV? both dr. Said different things.

First, nothing you have described makes me worry that you have herpes.  And if you did get herpes in this situation, it would most likely be HSV 1, which is the virus that causes cold sores.  have you ever in your life had a cold sore?  If yes, then it is extremely unlikely that you would then acquire this genitally.

1.  These could happen with a new infection, yes, they don't always but they could
2.  within a day or two
3.  YES, I believe anxiety would not cause all of these symptoms, but I think anxiety can cause us to notice symptoms that we normally would not notice at all
4.  Most do, yes, not all
5.  Again, the risk here would be HSV 1 from the cold sore virus, not really a risk for HIV
6.  The test for both HSV and HIV are antibody tests and there are a variety of types out there but avoid the IgM test for HSV.  A rapid test for HIV will give you accurate results and reduce the anxiety of waiting - if you can get it where you are.

I am not certain If I ever have had cold sores. 
 If I did I was a child and didnt know anything about them. 

Have you ever heard of anyone experiencing these symptoms (fever chills, tingling migrating  aches ) with out having lesions during a later confirmed herpe contration? Or is it safe to say that you will never get one without the other? 

Is the severity of hsv 1 genitally the same as hsv2 genitally? 

I've read it can take up to weeks for the sores to completely heal and go away. What is the shortest episode  from start to finish? Do the sores react in any way 
to anything like lotrimin af? 

My anxeity is through the roof and I cant get rid of the choking sensation on my neck. Dumb decision on my part.

I have heard of people having influenza symptoms without lesions and ending up with herpes but we could not know if they acquired herpes in that situation or later or earlier.  HSV 1 genitally is far milder than HSV 2 genitally - recurs less often and sheds less often
The longest I have seen an outbreak last is about three week.  The shortest would be a recurrence which might last three days.  They do not respond to Lotrimin
Herpes acquired genitally through oral sex would not cause anything in your neck at all - symptoms would only be genital.
I really don't think you got herpes at this encounter!

Thank you for your response, Im just super nervous about it. My last two question is at what risk, given the events during my encounter, am I to contracting HIV? Recieved unprotected oral sex and rectal fingering with possible saliva on the fingers. 

And what test specifically should I ask for, for HIV? I've read tests can be 90% convulsive at 4 weeks.

Hi, Joseph. Terri asked me to comment on the HIV issue you raise. You have already benefited from two follow-up replies from Terri, the normal standard for the forum, so my comments are brief. 1) HIV is virtually never transmitted by the sorts of contact you describe. 2) The chance a partner like your has HIV is very low in the US, so it is unlikely you were exposed. 3)Tthe symptoms you describe are not at all typical of a new HIV infection. I really don't think it's a serious concern.  I could understand if you were to decide to be tested anyway,  butI really don't think it's necessary from a medical/risk standpoint, only for reassurance.
