[Question #4105] hsv question

hey dr hhh hope all is well back here again, just a quick fyi from the last exposure i had, everything was negative besides by hsv1 was positive.

So this last weekend  Saturday was my birthday and my friends took me to the strip club, Monday morning i woke up with a few blister looking things( like 3 big and 4 small sores) on my thigh above the knee and after the shower i was rubbing my self with a towel and it kinda just scratched off and formed a blister yellow looking lesion.

i went to my primary care doctor that day and he said it doesnt look like herpes and and maybe it can be shingles so he tested me for hsv igm 1 & 2 both came back negative.

(few pointers) i also just moved that weekend and i was actually doing the physical moving with me and my friends. also had to do some minor renovation to do in the house before i moved.im also a married man =/

My last exposure that i had before this strip club i received unprotected oral from another woman i know didnt have any symptoms so never got tested and this happend like april 15 for the oral, and the strip club was aug 4 and the lesion happend aug 6 .At the strip club got a private lap dance for like 10 min touched her vagaina chest and but, she had like a bikini on and i was wearing long jeans and a t shirt.(i know i was told by you that you cant get anything if its not oral or physical sex so im not to worried about the stirp club)

And then i went to the dermatologist  Thursday Aug 9 and i showed him my thigh and  he said why would i get herpes on my thigh above my knee and not on my penis, but he said he will test me anyway so i made sure this time was an IGG test.my test results came back from lij lab hsv 1 possitive 53.50 >91 hsv 2 negative .10 <91.

so im pretty confident that i didnt get anything from the girl i got oral from with these results because its been a long time from april 15. aswell im pretty confident i didnt get anything from the strip club.

my questions are.

1.    can this be due hsv1genital since my igg test was so high?( I don’t know if I have genital hsv 1)

2.    Do I tell my wife anything because I didn’t have sex with her since my lesion?

3.    How did I do with the testing igm and igg and do I need to test again in a week?

4.    If it is genital can I transfer it to my wife? My wife has hsv1 idk if its oral or genital.

5.    How to tell if I have oral or genital hsv 1 rite now?

6.    If this lesion is from hsv1 would it come back?

7.    What should I do moving forward

1.    can this be due hsv1genital since my igg test was so high?( I don’t know if I have genital hsv 1)

Have you ever in your life had a cold sore on your lip or in your nose?  If yes, then you already have HSV 1 orally and you would be extremely unlikely to acquire this genitally

2.    Do I tell my wife anything because I didn’t have sex with her since my lesion?

If you have ever had a cold sore in your lifetime, I see no need to disclose anything to your wife about this.  it is clear that you have well established HSV 1 infection somewhere on your body.

So

3.    How did I do with the testing igm and igg and do I need to test again in a week?

I dI do not see any need to retest since the only risk I see is the contact in April for which you have already tested.

I

I

4.    If it is genital can I transfer it to my wife? My wife has hsv1 idk if its oral or genital.

If you are both infected with HSV 1, then I don't see a risk here for the two of you.

5.    How to tell if I have oral or genital hsv 1 rite now?

The only way to know the location of your infection is to have a lesion in either location and have it swab tested to see if it is herpes or not

6.    If this lesion is from hsv1 would it come back?

Yes, but not very often if genital.

7.    What should I do moving forward

If you have a genital lesion, have it swab tested.  You and your wife have the same antibody status so I don't see a risk here.

Thanks for the reply doctor,

Understood.

Just wanted to know as well if the legion i had was on my thigh above the knee is that considered genital or only genital is around your private parts?

Also worst case senerial if it was hsv 2 legion how acurate is  a igg blood test for hsv 2 when there is a legion present?
And i know a hsv2 is conclusive 12-16 weeks but wanted to know how long to wait after the legion goes away for acurate testing if i did want to test just to relax my self a bit?

Sorry and Thanks again doc

The area of the thigh above the knee is considered genital for purposes of the possibility of having a recurrent outbreak there.
The blood test doesn't care if there is a lesion or not - The result will be the same.  You do not need to wait longer to have a blood antibody test just because a lesion is present.
