[Question #4131] HSV-2/Herpetic Whitlow Risk Assessment

Hello, doctors.


I hate to return to the forum so quickly but I'm a 21 year old that had a worrisome encounter last night with this 18 year old girl and I really need a risk assessment. She was visiting from South Dakota and I really liked her so we were drinking and talking in the back of my car and we started making out And I gave her hickies on her neck and sucked on her nipple. My jeans and shirt stayed on and she was clothed as well but I did finger her  sticking my hands in her shorts for 10-20 minutes. She told me she wished she could have sex with me and I asked why she couldn't. She told me she caught genital herpes from her ex last year after i had been fingering her and sucking on her nipples. She was also on her period. There was no sex and all my clothes remained on. Just the kissing and fingering. I'm now worried I may have contracted HSV-2. What are the chances I contracted HSV2 genitally, orally, or in the form of Herpes whitlow on my fingers? Can you try and provide approximate risk ratios? Am negative for HSV1 AND 2 with igg test prior to this exposure. Havent had sex in months. heard Herpes Whitlow is incredibly rare is there a chance I caught it? Or hsv2 orally from kissing and sucking on her nipples? Please help :( I've read elsewhere mutual masturbation is no risk for infection.

If she has genital herpes, nothing that you did puts you at risk of acquiring genital herpes or oral herpes. And Whitlow is a very very low risk event also.  Just keep an eye on your finger and see if you develop blisters there but I would think you would not, if the skin on your finger was intact.  Sucking on her nipples and kissing presents no risk for you getting genital herpes from her.

Thanks for the solid information, Terri!

There was no cuts or anything on my finger. I also read that there's like 2 cases of whitlow for every hundred thousand people or something like that. Also that it would be harder to transmit to the finger than the genitald because the finger doesn't have a mucous membrane. you have like approximate numbers or percentiles of my chances of having gotten whitlow on my finger? Like 1 in 1000 or 1 in 5000? I've been using my left hand for everything now and I've been scared to touch my privates or my eyes with that right hand. And with herpes whitlow, would there always be an infection right away or is there a way it could stay dormant in the finger? Do you recommend testing for this at all? Or were my chances really that low and I'm overrreacting? It's day 4 and nothing has developed although I know too soon to say that.

If you had gotten whitlow, you would have symptoms. I have never heard of a whitlow infection being subclinical like genital infection can be.  You are slightly overreacting and I'm not being judgmental there - your word.  Keep an eye out for the next week or so and if no blisters on your finger, which I do not expect at all, move one and put this worry behind you.

Thank you Terri. That definitely puts me at ease. I hate to sound like a broken record but how low was the whitlow risk? Percentage wise or ratio wise. I know you can only give me approximate numbers and that theres lots of variables but given rhat there were no cuts on my hand, Would it be less likely then contracting genital herpes? I think it's 4% unprotected and 2% with condoms or antivirals. Should I avoid using this hand to masturbate or touch my face? I know this is my last follow up and thank you for providing such a valuable service. I respect you so much.

Oh Gosh, Terri. When I fingered this girl I used my right pointer and middle finger and I just noticed on my right middle finger there is a a little flap of white skin hanging off my finger. It's not red at all and I'm not feeling any pain. It doesnt really look like a blister but i have no idea how it happened and am assuming the worst. Am going to my dermatologist tomorrow for him to look at it. I'm deathly scared.

I would say it is way less likely than getting it genitally through intercourse - I do not have a percentage to give you. 
Herpes whitlow is truly remarkable - not a flap of white skin!
