[Question #4300] High risk HIV

Dear doctors,
I had unprotected sex vaginal sex many times with a girl I don’t know her status of HIV. The last sexual intercourse was on 6th of july 2018. I tested for HIV as the following:
- At 49 days (7weeks) 4th generation (Ag/Ab) combo test - Nonreactive
- At 58 days (8 weeks) PCR RNA HIV1/2 qualitative - Not detected
- At 78 days (11 weeks) 4th generation (Ag/Ab) combo test - Nonreactive
1- Please advice how conclusive is my results?
2- Should I wait till I complete 90 days and repeat?
3- Is there a possibility of my HIV status turn positive?
4- Do you recommend more analysis?

Welcome to the forum. Thanks for your question(s). It was unnecessary to post two questions, this one plus another asking about STDs other than HIV. I am deleting the second question and answering it here. I will ask forum administration to reimburse your credit card for the second question.
You say nothing about your partner by which to judge the chance she has HIV or other STDs. However, your HIV tests are conclusive and show for sure you did not catch HIV. Indeed, your very first test (the AgAb test 7 weeks after the last exposure) was conclusive; the other tests after that confirm the negative results, but were unnecessary. Although it used to be recommended that a final test be done at 3 months, that advice does not apply to the newer (AgAb, combo, 4th generation) tests, which are conclusive any time 6 or more weeks after exposure. Those comments answer all 4 of your specific questions.
In addition, the STD tests you report also are conclusive at the times they were done: Hepatitis B and C (two tests), RPR for syphilis, and urine chlamydia and gonorrhea tests, all done several weeks after exposure. (FYI, you were not at risk for hepatitis C. Contrary to popular belief, there is no signfiicant risk at all for sexual transmission heterosexually. You don't need any more testing for these or any other STDs.

In regard to the infections for which you were tested, it is completely safe to have unprotected sex with your wife. You cannot transmit an infection you don't have! Best wishes for a rewarding, recommitted marriage.---

How loong after the exposer the test considered conclusive for Hepatitis B, C and syphilis?
