[Question #4304] Genital hsv1

Hello doctors, I have posted in the past of about an exposure that occur 5 months earlier. After unprotected oral sex that lasted 2-3 seconds I began to have irritation after the encounter. That night I cleaned my genitals with dial soap. I was having irritation to the urethra. I was seen at an urgent care and tested negative for GC/chlmydia along with a negative urine culture. I was using lotrisone for 6 weeks without relief and causing the glans to become increasingly irritated and was instructed by my PCP to discontinue. My PCP felt it was not STD related erythema and I was seen by a dermatologist who examined my gland irritation he too felt it was due to the lotrisone and said to apply Vaseline. After stopping the lotrisone the erythema improved however remain present. The pain also improved. I am currently 3 months since the use of lotrisone. The pain persists I was on vacation and suffered an infection from coral reef to my leg and finished a course of cipro for the same. After I finish the antibiotic I noticed another area at the base of my penis that’s appears pink and well demarked about .5 cm with what looks like shiny tiny papules in the center and it does not hurt. About 5 days ago I felt pain base of my nostril, I though it may have been a pimp and popped the area it did appear clear fluid was released. The area is now crusted without pain.I recently started Valtrex being desperate for pain relief . I currently am taking 1g Valtrex BID I have taken 4 days so far. It feels like the pain has dramatically improved but the glans still appears the same before the Valtrex however maybe slightly better. I never had any ulcers that broke open. My question are
1. Given the pain relief does this suggest it is a hsv infection?
2. Did the use of lotrisone cause the herpes infection to last 5 months? I have no known medical issues that would suppress my immune system. However during this 5 months I was on short course 5 day steroids for Acute back pain.

While you may be having a cold sore at your nostril (have you had them before?), your genital symptoms sound absolutely nothing at all like herpes. Herpes lesions coming go and do not hang around for five months. Lotrisone would not have any impact on a herpes infection. Also, two or three seconds of Oral sex is really not enough to cause an infection and the vast majority of cases. I think that you are worrying needlessly.

I do not recall ever having a cold sore. Is it common not to have any blistering or ulceration with genital hsv1? Also is it common for hsv1 to be passed from genital to genital contact during sex? Your saying it very unlikely that after an exposure of that duration to get herpes, what are the odds of contracting hsv1 after 3-4 seconds of oral if you would have to estimate ?

Also does Valtrex have anti inflammatory properties I maybe benefiting from ?

It is uncommon for HSV 1 to be passed through intercourse, yes, very uncommon. And it would be very very unlikely to contract anything from 3-4 seconds of oral sex. Way less than 1 in 1000 I think
