[Question #4311] Follow Up to Question #4130

Hello again Ms. Warren,
I'm following up regarding Question #4130. I assume you're able to review it so I won't repeat anything we've already discussed. Since we last spoke, I took a 10 & 12 week HSV IgG 1&2 test. Results NEGATIVE. The 10 week test was significant for me because I went to a lab (Clinical Pathology Laboratories) that still gives results with a specific index #. The test returned with an index of (<.10) Obviously very low. I should be happy, but I'm not. I'm still experiencing a raw & irritated feeling on the skin around the top portion of my penis shaft & around my mouth, also my eyes seem to burn & are more irritated than normal. Still NO sores or redness in those areas. This all has continued for the past 3 months. I did get a sore on my butt around week 9, but it was an area that itched; & I scratched at it & squeezed it causing it to scab over. Not sure if it would have done so if I just left it alone or not.
A: It seems VERY unlikely that I got HSV from a single brief 10 minute unprotected encounter. B: Never developed sores on my penis or mouth. & C: Tests continually failing to detect antibodies to HSV a total of 8 times. It seems that the chances of all those happening together would be crazy low right? What are your thoughts? I'm having a hard time moving on. I'm probably going to pursue a Western Blot.
Full Testing History Post Encounter:
- (2 Weeks) Full STD panel including HSV 1&2 IgG plus HIV RNA early detection NEGATIVE (LabCorp)
- (4 Weeks) HSV 1&2 IgG Negative (Quest) & Urgent Care visit. Doctor checked lip & said it was not herpes.
- (5 Weeks) 2 Full STD panels including HSV 1&2 IgG NEGATIVE (1 @ LabCorp & 1 @ Urgent Care) Urgent Care Doc administered 1g Azithromycin & shot of Ceftriaxone.
- (8 Weeks) HSV 1&2 IgG Negative (Quest)
- (9 Weeks) HSV 1&2 IgG Negative (LabCorp)
- (10 Weeks) HSV 1&2 IgG Negative (Clinical Pathology Laboratories) Index titer of (<.10)
- (12 Weeks) Full STD panel including HSV 1&2 IgG (plus Trich) NEGATIVE (LabCorp)

My honest thought is that you are uninfected with HSV 2 and that you will likely need a western blot to be settled and reassured.
You have lots of evidence that you are uninfected but are not convinced.
The question is - if you do the blot (and your timing is OK to do it now), can you let THAT reassure you?

Yes, if I were to take the western blot now, it would be beyond 14 weeks, almost 15. I would definitely accept the results as fact. The test is considered 99.9% accurate and that is more than enough for me. It’s as good as it’s going to get.
I have already ordered the kit, and have an e-visit scheduled with you at 1:20pm on Tuesday October 2nd. This means I’ve done everything in my power to get the most accurate result. Case closed. Whatever result this returns is the final say. Fingers crossed. I look forward to speaking with you on Tuesday.

Frankly, I would have been more than happy to let this go and move on after all these test results. However, I have a persistent daily burning/raw sensation on my penis shaft, & a tingling, crawling sensation in my groin area. This has been happening for the entire 14 weeks from the encounter; but most intense for the last 10 weeks. Some days better than others, but still every single day. Besides the dry skin that appeared on my penis shaft after 7 weeks or so, I’ve had Zero sores or redness in any of the areas that I feel pain. I just need to know that it definitely is, or is not herpes. Hence the WB.
Questions:
1: Does this sound fungal, bacterial, viral?
2: Have you ever delt with similar sounding cases? What were the explanations?
3: Have you ever seen someone who isn’t on any antivirals test negative on an IgG at 12 weeks, test negative on a WB, who doesn’t have sores, and who has all these symptoms?

we've talked now Jeffrey. As you know, I don't think this sounds herpetic to me. When a person's only symptoms are burning and raw feelings, the vast majority of the time, they test negative for HSV 2. And herpes is not an "every day" thing either. And most definitely I have seen people test negative on the WB at 12 weeks - the majority of people in your situation test negative on the blot! But we shall see. I am very hopeful

Since we spoke on Tuesday (after I posted my most recent questions); I was not expecting you to answer them, but thanks anyway!
Final Follow-ups:
1: As of Tuesday October 2nd, I started taking Fluconazole 150mg once a day (10 Day Supply), Cetirizine (Zyrtec) 10mg Once a day (30 Day Supply), Ranitidine (Zantac) once a day (30 Day Supply), and using a 2% Ketoconazole Cream twice daily. Will any of these have an adverse affect on the WB?
2: Since we spoke on Tuesday, I've yet to receive the pre-filled out Lab Requisition Form for the WB from you. I was wondering when I could expect that?
3: Not super keen on my name being used in that last reply from you. I know it's just the first, but is there any way to remove it? Thanks!

I sent it once again! That's three times. Did it maybe go to your junk mail? Please check the email that you used on evisit and if incorrect, say so here and change it (don't list it here) and I'll go into evisit and try sending it again.
