[Question #4334] Was i at risk with a shaving cut, and 4th generation lab test question?

Dear doctor . I had a small shaving cut on my cheek bone 12 hours before the strip club visit. It stop bleeding then i applied triple ointment and let the car air conditioner on it, later applied  bandaid but it fell off. Cut was exposed with no bandaid during lap dances. I'm concerned if sweat (if there was any) as girls rubbed their breast into my face, coulda gotten into my cut. Also concerned when one of the girls dance on top of my head hair for a bit, if any vaginal fluids ( if there was any) coulda gotten into my cut.  I don't recall feeling any stinging on my cut, so i assume there was never any sweat or vaginal fluid making contact with my cut . I took an Hiv 4th generation lab test blood drawn from arm vein  at exactly 48 days  done at a lab, and my result was NEGATIVE. 
1 - Is it possible for an Hiv 4th generation Negative lab test  done at 48 days to change to positive later on? 
2 - Is my NEGATIVE 4th generation test 100% percent conclusive at 48 days, or do i need more testing?
3 - Is it possible for outside dripping vaginal fluids to transmit hiv through a shaving cut?
4 - Is it possible for a persons  sweat to transmit hiv through a shaving cut?
5 -  And is a twelve hour small shaving cut good enough as heal to protect and avoid such thing?
6 - Also does ejaculating in once pants during a lap dance a risk, can hiv go through clothes? 
7 - Was i even at risk to begin with?    8 - Is the new CDC conclusive guideline really 45 days for 4th generation?

Welcome to the forum. Apologies for the longer than usual delay in responding.

Nobody has ever been known, reported, or even suspected, to my knowledge, of acquiring HIV from exposure of an existing cut or wound. Among medical personnel who become infected through an injury, every case has occurred because of an injury by an HIV contaminated instrument, usually a hollow needle or an obviously bloody sharp instrument like a scalpel. Even if one of the lap dance women had HIV, you were not at any risk at all. (And probably there's under 1 chance in a thousand that such women have HIV anyway.) Finally, the HIV AgAb (4th generation) antibody tests are 100% conclusive any time more than 6 weeks after exposure. So even if you had been exposed, your negative result at 48 days proves you were not infected.

Those comments pretty well cover all your questions, but to be explicit so there is no misunderstanding:

1) No, not possible.
2) 100% conclusive, no need for additional testing.
3) "Possible"? Probably not. But even if theoretically possible, it has never been known to happen. Zero risk.
4) Sweat does not contain HIV and is not known to ever be a source of transmission of the virus.
5) Probably yes. But this doesn't matter. We don't fully understand the biological reasons that such exposures are risk free. We just know that they are, so the exact reasons don't matter.
6) No risk. Neither HIV nor other STD bacteria and viruses can pass through clothing. Anyway, ejaculation or not does not change a man's risk of catching HIV, even with vaginal sex.
7) Nope, not at all.
8) Yes; see above.

Have only safe sex and/or sex only with partner's at little or no risk, and you will never catch HIV. Do not worry about any other kinds of exposures, in strip clubs or anywhere else.

Thank you Soo much!!  Dr.Handsfield you are a true legend!!  I read alot of your work on the old forum and on the internet, plus youtube videos. You are the best on helping the worried people on cases like this.  Must admiration for you and Dr. Hook as well.  I always used a condom when i'm with a girl.  The only time i slipped was two years ago in 2016.  I wind up getting a Uti thing with something unrelated to this incident with a girl, but of course i panicked out and thought the worse. That's when i started to surf the web and found the old forum and your good advice. None the less That time i tested myself at 3 months and even at 6 months  at the 180 da mark , both  with 4th generation antigen/antibody lab test  (like the test i had recently)  and my result was also NEGATIVE.  Ever since i guess i develop a phobia to it and i try to take care of myself the best ways i can. From washing my hands often  to always using condoms, and avoid sleeping around. I prefer to be in stable relationship with a women. I guess i can get paranoid about hiv and everthing . I'm glad the CDC change the window on such a good test like the 4th generation. You expert  specialist doctors prove to be right and are always right!!!!  I'm hiv Negative and will always practice safe everything to remain NEGATIVE.  Once again thank you Dr. Handsfield :) : )

Dear Dr. Handsfield.  Just for knowledge i got a quick little question regarding vaginal fluid.   I read before that usually the infectious vaginal fluids that infect a person with hiv  are you usually deep in the cervix area of the vagina. Is this really true?? And if so why??  I read other post elsewhere where they consider outside vaginal fluids in the libia same as sweat and they say no risk at all.    But i figured i ask your expertness  specialist doctor.   Looking forward to your answer my good doctor.  Once again thank you very much for helping me a week ago. I'm stress and anxiety free now.  I'm hiv NEGATIVE and will do everything safe to remain NEGATIVE

In general, I advise people concerned about HIV risk to ignore the biological reasons why certain practices are lower or higher risk. There is intellectual interest, of course, and it may be satisfying to know the biological explanation. But the important thing is that whatever the reasons, sexual transmission of HIV generally requires penile-vaginal (or -anal) sex. Anything less intimate is risk free for practical purposes.

Having said that:  the lining of the vagina is quite similar to skin, with squamous epithelium that has few or no cells infected with HIV and no glands that secrete fluids that might contain HIV. The lining of the cervix (and perhaps the uterus above) is the only source of HIV shedding in the female genital tract. Therefore, it is logical that the concentration of HIV is highest in the cervix, or secretions emanating from the cervix, and lower in the vagina itself, labia, etc. This likely helps explain why HIV transmission requires vaginal penetration and that genital apposition -- i.e. genital contact without penetration -- carries little or no HIV risk.

Thank you once again Dr.Handsfield.  I guess to finish the thread i'll ask my last question once again just for knowledge.  I was wondering is it possible for Hiv to be transmitted at a barbershop.  For example if the barber winds up cutting you a bit with the hair clipper machine or cutting pop a tiny small pimple or eczema sore with it, and it starts bleeding just a bit.  Lets say the hair clipper machine wasn't disinfected  ( tho my barber say he always disinfect them by law, show me the cleaning spray thing bottle )  but if it wasn't  and let's say the last person who got their hair cut was infected.                                                                                                                                                                    1 - Is this any possibility for Hiv transmission if lets say dried up blood was on the hair clippers razor?                                                                                                                                                    2 - And how long does Hiv virus last outside of the body host??                                                                                                                                                                                                                                          3 - I hear it dies almost immediately is this true??                                                                                                                                                                                                                                                                     4 -Is any Hiv testing required after an  incident of a small barbershop cut that bleeds a bit??                                                                                                                                                                        I know phobia and Anxiety are getting the best of me i guess, i know alot of threads answers of experts  say NO risk at all and i really shouldn't be worried.  But i would like to hear it from your Expertise, you always explain things better and really help people get over there fear and Anxiety. In theory, HIV could be transmitted in a barbershop. For example, if shaving and cutting an HIV infected person and then immediately shaving another person while the blood was still wet, and cutting him. If you think about it, this scenario is exceedingly unlikely and probably has never occurred. There might be slightly higher risk for other blood borne infections, like hepatitis B or C, and of course all barbers should disinfect their scissors, razors, etc between customers. But if they forget or just don't do it, the risk is very low and probably zero fo HIV. There has never been a reported case of HIV acquired in barbershops, beauty parlors, etc. (Unless someone had sex or shared needles while there!)

1) No realistic possibility.
2,3) The virus rapidly dies when blood or infected fluids dry out, but nobody really knows exactly how fast. But this really doesn't matter anyway. Since such transmission doesn't happen, who cares about the biological reasons?
4) No HIV testing is recommended in this situation.

As for following "all my advice", re-read my comment in your opening question ("Have only safe sex...?).
