[Question #4346] Epididymitis

Docs, I'll keep it brief as we've already discussed my exposure (oral and analingus with CSW). My left testicle has been aching for over a week now. It's a dull ache, but persists all day, enough to be annoying and concerning. I am able to touch the epididiymitis and it doesn't send shocking pains or cause me to jump. The discomfort seems to be coming from the bottom of the testicle, but I can't be positive. The area does not seem more warm than usual, and my scrotal skin is always somewhat red. I should note that I have a varicocele, but I've had that my entire life without this type of pain. And I also have had an inguinal hernia for 5 years, BUT it's on the right side, not the left where this pain is localized. My brief questions: 

1)    In men with epididimytis, would feeling the epididymis cause a strong reaction of pain? Is it more than a dull ache and would one feel the sudden need to seek treatment? Would it have been gradually worse upon lasting an entire week that I would have no doubt about infection? Would dipstick urinalysis signal infection if the testicles were infected?

2)    Has epididymitis ever been known to spread through oral sex or analingus?

3)     Are there any other reasons for unilateral testicle pain that I should be considering? (My urine std tests have been negative, just concerned about any other cause through bacteria passed into the urethra or anus).The CPPS symptoms I have experienced over the past 6 months have never included a testicle. 

4)  If, for whatever reason, the CPPS pains I have been feeling were due to some bacteria   from my dalliance, is 6.5 months entirely too long for it to spread to the testicle? 

 5) Is my partner at risk if this could be a testicular infection? Are antibiotics even warranted here, and at what length of time should discomfort persist before seeking treatment?

Sigh. You keep returning for reassurance after an exceedingly low risk exposure. Your current symptoms, as your previous ones, do not suggest anything physically wrong and I am very confident you do not have epididymitis. As I probably said in one of more of your previous threads, these symptoms are most consistent with anxiety magnifying minor symptoms or even normal body sensations that otherwise would not be bothersome and maybe not even noticed. 

1) Epididymits typically causes obvious enlargement and exquisite tenderness of the testicle, not just aching. 

2) Sexually acquired epididymitis due to gonorrhea or chlamydia could be acquired by oral sex, but analingus is unheard of as a cause. Anyway, you were tested and negative for both chlamydia and gonorrhea.

3) We discussed prostatitis and CPPS in one or more of your previous threads. They could cause such discomfort. But I'm still betting on the emotional/psychological explanation.

4) This is not a plausible scenario at any time.

5) As we discussed previously, I remain confident you have nothing that can ever harm a current or future sex partner.

I've previously asked you not to keep posting about this question. This is definitely going to have to be your last. As I may have said previously, the forum does not permit repeated questions on the same topic or exposure. Future new questions about this exposure and these symptoms may receive no reply and the posting fee will not be refunded.
