[Question #4389] Exposure Risk From Oral Sex On A Dancer

I unfortunately made a horrible mistake and drunkenly performed oral sex on a stripper for what could have been 5-10 minutes. I did not notice any visible signs of anything but don't remember looking. Since then I have not really been able to sleep or eat out of guilt and anxiety. I have read just about any and all posts on med help and other sites but wanted to confirm if there is any new information specifically for my scenario. I have been feeling some symptoms since then (day 2) with a sore throat and some sensations on my lips and tongue, and also a periodic sensation of "hot around the collar" sensation, but am hoping that they are all from anxiety, I have not noticed any lesions. I went to my Dr. and had my throat swabbed for strep and it was negative but she did give me, at my request 1000 mg azithromycin and 400 mg cefixime in case I picked up gonorrhea or chlamydia or syphillis . I went and had a baseline std panel to make sure that I still have no previous infections (tested negative for everything in 2015), should have results in 7 days.
1. What are my risks of exposure from this encounter (oral to vaginal only for 10 minutes) for anything especially incurable? I am concerned about everything but from what I read my main concern i guess is HSV2 contracted orally. How long with no signs of any sores would I be able to feel confident that I have not contracted anything?
2. How long should I adhere from relations with my wife or even kissing contact or sharing cups or anything with my kids. I really could not live with myself if something happened to them from this terrible mistake. Should I ask my Dr. for any antiviral medication in case I have contracted the HSV? I really want to make sure i have

I hear that you are incredibly anxious about this encounter. The risk of contracting HSV 2 at this encounter is really really low. First, she would need to be infected with HSV 2 and that's maybe a 20% chance. Then, she would need to be shedding virus at the exact moments that you were giving her oral sex - you said you noticed no lesions so it would need to be asymptomatic viral shedding which happens, if she happens to be infected, on maybe 15% of days, with most episodes lasting less than 6 hours. In addition, HSV 2 does not like the mouth and uncommonly shows up there. You've seen no lesions which is great. And the symptoms that you are experiencing could certainly be attributed to anxiety (particularly the "hot under the collar" thing. If you take all of these factors together, I think your risk is just incredibly low!

Exposure was just over 3 days ago.
I read in one post that you stated in 33 years that you have not seen a case of oral HSV2 that was transferred via oral sex from a man to a woman. Is that true?
I have not noticed any lesions, I am constantly looking and noticing different things that frighten me and think they will turn into lesions but I'm hoping that is the anxiety again. If I were to develop any lesions they would be obvious and painful correct?
What other STD/STI should i be concerned with? I know Dr. HHH stated over and over in previous posts over the years that there was virtually no risk for most other STD/STI and actual zero risk for HIV from this type of activity. Is that still the case? He usually would end with, if i found myself in your situation I would not seek additional testing and would continue to have regular sex with my wife. Would you feel confident in telling me the same?
I had planned on follow up testing at 6 weeks and 12 weeks respectively for everything just to make sure, do you think that is wise or unnecessary?

Your risk for acquiring HSV 2 from receiving oral sex is practically non-existent. I thought you were worried more about HSV 1. If you are going to develop symptoms, it would be withint 2-10 days so you are 1/3 of the way
I would agree that the risk of HIV from receiving oral sex is zero. There is a very very tiny risk that you could acquire a different STI like gonorrhea or chlamydia from receiving oral sex, as Dr. Hood has mentioned in previous posts.
I try not to give advice about continuing to have sex with a spouse, but in this situation, infection seems very highly unlikely. And you have been treated for chlamydia - the treatment you were given to cover gonorrhea is not the current recommendation but is probably fine.

To clarify...I was the giver of oral and she was the receiver. I am concerned that I would contract HSV2 or anything else orally from performing cunnilingus for the 10 minutes.

I really just want to understand my odds for getting any and all STD/STI and what would be most likely so i can try and make sure if i can, to mediate those risks with medication.
From the cunnilingus what are my risks for the following?:
HSV2
HSV1
Chlamidia
Goonereah
Syphillis
HPV
HIV
How long should i steer clear of any acts that would potentially put my wife or children at any risk. Should i get tested for anything and if so when?

The odds of getting any of those infections by giving oral sex are all very very low. Especially low are the two herpes viruses but we've also not seen cases of HIV being transmitted via oral sex either. Have you been tested for any of these sexually transmitted infection since the encounter? How long ago was the contact?

Incident happened on 10/13
Baseline tested on 10/15 (all negative), took 1000 MG Azithromycin for sore throat
10/16 Took 400 MG cofixime for potential infection (trying to get out ahead of anything)
10/18 Took another 1500 MG Azithromycin in case of syphillis exposure
10/20 Had sex with my wife, sore throat was gone
10/22 (day 9 after exposure) still feeling burning sensation on my lips, very dry constantly using balm, periodic headaches. I have not noticed any lesions as of yet that i know of (would they be glaringly obvious?)
How long do I need to wait until i can feel fully confident that i have not acquired any infection at all? I have tried to take preventative steps in order to eliminate some infections i could have picked up with the antibiotics. I guess at this point the only thing realistically that i am at risk for still would be HSV either 1 or 2 or HPV. Would a HSV swab test be of any use even if i dont see any noticeable lesions? I know that the blood test wouldnt be conclusive until 12 weeks but that is such a long time from now. I really need to put this behind me!

No, without a lesion, I don't believe that a swab test for herpes would be of use now. I wish there was some magic way to make the time pass more quickly but unfortunately, there is not. I can only tell you that I think your risk was incredibly low to start with and then without the development of any lesions, still lower. I don't believe that dry lips have anything to do with herpes and periodic headaches could be anything. And yes, as worried as you are, I believe that you would notice any kind of lesions.
