[Question #4412] Oral Exposure

Hi doctor. Thank you for your valuable advice and help.
Like so many others, i had my first and last oral/fingering exposure on 30 July 2018 with a girl (Filipino, 23 yrs old) in Dubai, UAE. No ejaculation or blood (i believe). She tested negative after 30 days in an obligatory residency blood test (not sure what HIV test they use - but i know that in Saudi Arabia they use 4 g in such occasion).
No major symptoms i believe: Sore throat for 3 times - normal one for 1 day, normal one for 3 days, and one for almost 12 and was painful a bit but the doctor told me my throat is clean and this is due to whether allergy only.. noting that my car AC was not working that time.
I stopped brushing my teeth due to stress and i got receding gums after almost 2-3 weeks and blood when i brush until i got it fixed by the doctor (i used to stop brushing before and had some blood when i brush but never had receding gums). In addition to Bruxim in the same period.
I can feel my nodes under cheecks but not enlarged at all (probably because of gums) for around 40 days now (not obvious or painful and doctor also told me due to wheather).
Very small white spots i can see and feel on mouth angles didnt increase or decrease.
1- Do those symptoms suggest any infection? I see that being overstressed is a reason but would like to know from experts like you. Googling kills!
2- Is it really possible not to develop symptoms at all?
3- In case there is is no blood in receiver mouth, is it still risky?
4- If the test she did is 4 g, do you advise me to test?
Thank you in advance doctor.

Welcome to the Forum. In reading your description of the encounter and history since then, my sense is that you already know this was a no risk encounter but the you continue to worry, and that perhaps despite knowing better, you have been on the internet looking for information. I'll try to help.
First, let me point out that there has never been a proven case in which someone acquired HIV from receipt of oral sex - not ever. Whether or not she had oral sores, bleeding gum, etc. makes no difference. Further, the fact that she had a negative HIV test 30 days after your encounter, no matter what kind of test, PROVES that she could not have given you HIV.
Isolated sore throats are not suggestive of recently acquired HIV.
Thus, the brief answers to your specific questions are:
1. No, none of the symptoms you describe suggest HIV or any other STI.
2. Theoretically, yes, it is, but nothing you have described suggests any risk.
3. With or without blood, this was no risk.
4. I see no medical need from your description to test.
Although I suspect you already know it, I would also advise you to stay off the internet for questions of this sort. All too much of what is there is misleading and incorrect.
I hope these comments are helpful. Take care. EWH

I sent it by mistake.
I was in a bad situation until i started reading your replies to different cases.
My last question in order no to annoy you with repeated questions:
1- even if it's only antibody test that has long window period, is the 30 days considered enough?
2- The problem is that we google in big websites and thats what misleads us (they say swollen gums is a symptom). But i prefer talking to expert like directly instead.
Thanks again for your valuable coments.

In general, Even a negative antibody test at the time of a sexual exposure is very, very strong evidence that the person you had sex with was not infected. In your case, that the testing was done 30 days after the exposure absolutely assures you that she was not infected at the time you had sex.
Whatever "big" web site you found is wrong. Unfortunately, some of the most widely encountered websites are distributing incorrect information. EWH

Before ending the thread doctor:
1- Can lymph nodes be invisible (as mine) in HIV or should they be enlarged with fever? They are not visible at all but i can just sense them when i touch since alkost 40 days.
2- If no blood in mouth, how is it possible to acquire the virus when the it doesn't live in mouth as saliva and air (while breathing) kill it?
Thank you so much doctor Hook for your valuable time and responses. Reading your responses is what made me feel better and confident really.
Best regards,

1. Persons with HIV do not need to have symptoms at all. Many persons with swollen lymph notes do not have HIV and, conversely not all persons with HIV have a swollen lymph nodes. The appropriate way to diagnose HIV is with a blood test. In your case however, nothing you have written about suggests any risk for HIV or any medical need for testing.
2. Your question is a bit hard to follow. HIV is not transmitted through oral contact. Not through kissing and exchange of saliva, not through receipt of oral sex.
I hope the information I have provided has been helpful to you. As you know our forum limits responses to clients to a total of three answers. This is my third response to your questions. Therefore this thread will be closed shortly. Please don't worry about the encounter you describe. It absolutely did not put you at risk for HIV. EWH
