[Question #4436] CSW Exposure Risk.

Hello Experts,
After days of scary googling and pictures I am thankful to have found your site.
3 weeks ago I had protected intercourse for a couple of minutes at a massage parlor. (This is where it gets messy) after intercourse she took off the condom and gave me a very rough hand job. This included firm rubbing of the penis head and massaging the inside of my butt crack. (She used to same hand that she used to take the condom off so her vaginal secretions were obviously on her hand) After a few minutes of this we have proctected sex once again for maybe a minute and a half. Unfortunaley this second try when she put the condom back on I was not fully erect so during the end I noticed the condom had slipped and was on maybe half way. After noticing that I freaked out and ended the entire thing.
Since maybe the second or third day I have been experiencing some strange symptoms Im hoping you could help me diagnose.
My penis head has been having some sharp on and off again pain. It burns when I pee.
Everyday since than I've been having pain while going number 2. It mostly burns. I have cut down my eating to once a day on account of I'm afraid of going to the bathroom. (Could this have been caused by her finger rubbing me? It never fully penetrated my anus, but it did have her secretions on it)
I also noticed something that resembled a bug bite on the side of my knee that I never would have thought much of before. Its since crusted over with a hair coming out of the middle part. Not sure if this is related but that has starting to cause both knees to start itching.
Any idea if this sounds like an STD? and would STD symptoms last 3 weeks straight?
I have no idea of what tests to be tested for other than chlamdeia and gonnoreha. Which I did last week and they came back negative. Do I need to test for Hepatitis, HIV, Herpes, and or Shyhlis? Can you quantify STD risk?
Thank You.

Welcome to the forum. Thanks for your confidence in our services. Your question came in while I was on the forum -- most users shouldn't expect nearly real-time replies!

First, congratulations for using a condom for both episodes of intercourse. That's by far the most important aspect of these events and protected you substantially (if not perfectly) from any and all STDs. Second, remember that even among the highest risk partners, including sex workers, at any time most have no active, transmissible STD, with the possible exception of HPV. Third, hand-genital contact, including the details you describe -- like vigorous contact, hand contact with other body areas including genitals and anal area, or use of genital fluids for lubrication -- rarely if ever transmits STDs. So all things considered, this was a low risk exposure in regard to all STDs.
It is natural and normal for condoms to slip part way down the penis during sex. Intact skin is not commonly infected, and condom protection is nearly complete as long as the head of the penis and meatus (urethral opening) remain covered. The slippage could have increased the chance of catching one of the STDs transmitted skin to skin (herpes, HPV, syphilis) but not of those transmitted primarily through sexual fluids (e.g. gonorrhea, chlamydia, HIV, hepatitsi A and B).
As for your symptoms, they are nonspecific and not typical for any STDs. Among other things, neither STDs nor irritation from vigorous exposure would continue unchanged for 3 weeks. I have to wonder whether part of the problem is magnification of minor symptoms or even normal body sensations that you wouldn't notice except for your obvious worry about STDs or perhaps a sexual decision you regret. However, since they have lasted this long, it would make sense for you to be professionally evaluated. Ideally you should see an STD expert, such as a health department STD clinic or a Planned Parenthood clinic. (If you see your personal physician, do not hold back on the details of the sexual exposure and your symptoms.)
The "bug bite" sounds like typical folliculitis, i.e. inflammation around a hair follicle. It is unrelated to the sexual events you have described.
Your negative gonorrhea/chlamydia test results are conclusive for genital infection and do not need to be repeated. If you pursue medical evaluation, blood tests for HIV and syphilis would make sense (at 6+ weeks), more for reassurance than because of any substantial risk. I see no need for testing for viral hepatitis or any other infection, but follow the advice of the doctor or clinic.
I hope these comments are helpful. Let me know if anything isn't clear.

Thank You Dr. That does make me feel better that you consider it low risk!
Also thank you for the detailed answer. You stating that std symptoms do not last 3 weeks straight is also comforting. I forgot to include that I am not circumsied not sure that it matters since there was condom use.
I suppose that if the symptoms don't get better over time I will see my Dr. Would you recommend that I obstain from sex until I see a Dr? There is a signifigent other so I don't want to chance her catching anything.

It isn't possible to be 100% certain you have no STD from the events described, but the chance is very, very low. If I were in your situation, I would continue unprotected sex with my wife without fear I acquired anything I could transmit to her. But I'm not you and you'll have to make your own decision.---

Thank you Dr. That is something I will have to think about. My last question is .. and this is all so to google. I really wish I had not looked up Anal herpes before I had found you. Is this a pretty low chance of anal herpes? Does it even fit the symptoms? Besides the burning. There are also times of itching. Though not consistently. Also times that there it feels like something sharp back there, but once I clench it goes away. Almost like chaffing. Sorry for the tmi.

There is no realistic chance this is anal herpes. As for other STDs, herpes is rarely if ever transmitted by hand-genital contact and your anal symptoms aren't right (not severe enough, too long lasting, no painful sores). The symptoms are more suggestive of an anal yeast infection, which is not sexually transmitted but might be stirred up and become symptomatic upafter by anal fingering. You could try local hygiene (washing with soap and water twice a day) plus an OTC anti-yeast cream like clotrimazole or miconazole. Or just follow through with professional exam and follow the advice you receive.

That concludes the two follow-up comments and replies included with each initial question, and so conclude this thread. I hope the discussion has been helpful.
