[Question #4606] How worries should I be?

I recently (about 2 days ago) had unprotected vaginal and oral sex with a stripper. I have zero background on her.  Is HIV a real concern for me here?? Thanks Doctor

Welcome to our Forum.  Let me give you some generalizations to react to and let's see where it takes us.

Your overall risk for HIV and other STIs is rather low.  Most people, even most commercial sex workers do not have STIs and when they do most STIs are not transmitted through a single exposure.  How frequently they are transmitted however varies with the infection. For instance, HIV is transmitted from infected persons to uninfected persons about once in every 1000 sex acts, on average.  Gonorrhea on the other hand is transmitted to uninfected partners about once in every five exposures.  Thus, on a statistical basis your risk is rather low and you will probably not be infected through a single encounter.  On the other hand, your risk is not zero.  Depending on just how nervous you are, you could test for the most common STIs (gonorrhea, chlamydia) now and get reliable test results.  You will have to wait to test for HIV- the most widely tests are not conclusive until about six weeks after exposure.

Were I you, I probably would not worry too much but I would seek the reassurance of negative tests for gonorrhea and chlamydia.

I did take a chlaymydia and gonorrhea test today.  Should have results tomorrow / Wed.  I also went ahead and sprung for and HIV RSA Antigen test.  I can  take that at 11 days and they are telling me it is 95-99% accurate.  Your thoughts? Should I test for syphilis?

You are correct, the HIV RNA test will provide strong evidence of the presence or absence of infection after day 11.  Expensive though.

Syphilis is so very rare among women at this time, I really would not be worried.  EWH

One last question question here.  I do remember most of the night.  I know I DID not ejaculate.  I had not had an orgasm for over 24 hours and since I was drinking heavily I was urinating very often.  Is there any real chance of pregnancy here?

The risk of pregnancy is low but not zero.  Ejaculation is not required for pregnancy to occur. that said, this is not something I would worry about.
