[Question #4730] Anal sti/prostate/semen

I went to a massage place and received a hand job while the masseuse licked my anus and perineum for a few minutes. I didn't think anything of it at the time, but have experienced pain in the perineum, anus, groin and genitals over the course of several months (11). There never was any anal discharge, itching, or loose stools. Just pain. I decided to take a urine STI test at 6 months post exposure since I had slight penis tip irritation post-urination, which was negative. I went to a doctor and he diagnosed "prostatitis," but he didn't believe I had an infection. I did not tell him about the anal licking out of embarrassment. My questions:
1) Can licking of the anus and perineum put me at risk for an sti? How often have you seen this in practice?
2) If so, and it remained untreated over the course of 11 months, could it migrate to the prostate and take residence there until treated? Would an infection of this region clear up over this time period by the immune system?
3) If infected, would my ejaculate fluid contain bacteria, which would presumably infect my wife? (We have had sex many times and she does not show signs of infection).
4) If there was in fact an anal/prostate infection, could it migrate to the urethra or testicles? I have read that it's a possibility, but could be wrong.
5) Finally, I am getting a semen analysis done for fertility purposes. I am terrified it will be abnormal because of this. Should I be concerned here? If there was indeed an infection causing my symptoms, I'm concerned my wife could just be asymptomatic but this test will expose an infection from the above described event.

Welcome to forum. I'll be glad to comment. The exposure you describe was virtually no risk for STI and is not something that would be associated with the sorts of anal and perineal discomfort that you have described. To elaborate by addressing your questions:
1) Can licking of the anus and perineum put me at risk for an sti? How often have you seen this in practice?
While there may be a theoretical risk of infection with such a practice, the fact is that there is no appreciable risk. I have never seen or heard of an STI being acquired from either massage or anal licking.
2) If so, and it remained untreated over the course of 11 months, could it migrate to the prostate and take residence there until treated? Would an infection of this region clear up over this time period by the immune system?
Again, no. Prostatitis is rarely, if ever an STI.
3) If infected, would my ejaculate fluid contain bacteria, which would presumably infect my wife? (We have had sex many times and she does not show signs of infection).
Some persons with prostatitis due have their disease due to bacterial and in those cases bacteria are present in the urine and/or prostatic fluid. OTOH. much prostatitis is non-bacterial. Prostatitis is best diagnosed and managed by a urologist.
4) If there was in fact an anal/prostate infection, could it migrate to the urethra or testicles? I have read that it's a possibility, but could be wrong.
Again, see above. Please don't be worried that this is an STI or that there is now risk for spread to the testicles or urethra.

Infection and/or inflammation would be detected in a semen analysis. I would not be worried about STIs however.

Hello Doctor,
I'm coming across a bit of wait time for the semen analysis, so I will have to reply at a later date if the results warrant it, irregardless of the additional fee. However, I am having pain right after urination on a fairly consistent basis that is concerning. It will usually last a few minutes after urination and then resolve. It does not hurt at all during urination, just immediately afterwards. There is no discharge. I have taken a few of the Siemens urinalysis dipstick tests, with both first void and end of urination samples and none have shown nitrates or Leukocytes. I do take comfort in your previous reply, and I thank you for that, my questions:
1) Is there cause for concern with post-urination pain/stinging months after anal licking by a csw? What could cause this and will it resolve on its own?
2) If there were indeed a std infection in my prostate, or anal region unresolved which was causing this post-urination pain, would the dipstick urinalysis at least show nitrates or leukocytes?
3) Is it logical to think that if there was indeed an infection, it would colonize the urethra and show on sti test?
As a reminder, I have taken a urine std test for gin/chl and they were negative, out to 6 months post exposure. I'm just concerned that this painful post-urination is an indication of infection which is coinciding with a significant deal of prostate/perineum pain. Any and all symptoms started after this event, but it has been 10 months and they're still present daily.
My apologies if a portion of this is deemed repetitive.

thanks for reaching out. We can keep the thread open so you can post your results. Your report of discomfort which is most prominent at the end of urination (so-called "terminal dysuria" is not typically a sign of STI of any sort. On to your specific questions:
1. See above. This is not an STI symptoms. I would ask a urologist but it does not raise concern about any STI.
2. If this were an STI, it certainly would be possible that the dip-stick would show leukocytes or nitrites.
3. Yes, absolutely

I’m afraid my semen analysis is going to take another week or so, and this thread will be closed by then. If results warrant, I’ll repost.
My last question on this thread, regarding the same exposure and fears of symptoms that my wife is experiencing. She is defacating multiple times per day, much more than usual. It does not appear to be diharrea though, and she often complains she feels like she has to go and her stomach doesn’t feel right. She has never complained about vaginal symptoms. She does not complain about the lower abdomen where reproductive organs lie.
1. Can any sti cause gastrointestinal symptoms such as described above? If indeed I had an infection of the prostate related to this event, and we have unprotected sex, is there reason to be concerned that my semen could be the root cause of this stomach distress? Or is the vaginal canal and stomach biologically separate and there is no reaction of this type from a male’s semen?

1. Your wife's GI symptoms are not those typical of any STI, particularly not anything that would be spread through vaginal intercourse. As I believe I mentioned earlier, prostatitis is typically not caused by STI pathogens and when bacteria cause it, they are not typically spread through vaginal intercourse. I suspect your wife's GI problems are unrelated to your prostate problems.

Thank you for keeping this thread open. My semen analysis for fertility came back normal, and I even took an anal swab test, along with a urine test for chl/gon with both being negative. I am thankful I can begin the process of moving on. Thank you for you services here and your help!
