[Question #4748] Herpes or BPH type infection

I’m a male 53 years old and uncircumcised.  40 days ago I was recipient of  unprotected oral sex from female for couple minutes then sexual intercourse using condom.   Week later after sexual encounter I went for blood test and results said I had herpes 1 and 2.  About 12 days later I felt itching at top of penis. And some itching in groin area.  Itching comes and goes and I have ache in right testicle.   I then went for examination and provider saw no signs of STD.   Provider said itching could be psychological.  My lip cold sores were never diagnosed.  Provider said if previous blood test were positive its possible I always had HSV2 on lips.  Few days ago I started applying  clotrimozale cream on penis head for itch and base of penis. I never had a actual outbreak. Itching is a little better.  Testicle pain is intermittent.  Is it possible I always hadHSV2 on lips or is this possibly related to BPH related infection from dripping urine in underwear causing dry skin in groin area?

Can you please post here your HSV 1 and HSV 2 index values (numbers associated with the positive value) and was it and IgG test or IgM. 
How long have had you had cold sores on your lip?
I think it HIGHLY unlikely that if you truly do have  HSV 2  on your lip. 
And testicular pain has nothing to do with herpes but if you have testicular pain, you should have that checked out for sure.
I'm curious about why you think you might have prostate problems?

I’ll probably have to wait till Monday to find out index values and I don’t know what type of blood test I took.  ( believe IGg) I’ve been sexually active last 40 years since I was 15  years old. Cold sores since I joined military at 18 years old.  I have been diagnosed with BPH .  I have enlarged prostrate.  The urologist did a cystoscope and wanted to do surgery.  I chose surveillance. That’s why I have a dripping issue after I go pee.  I’ve learned recently to take my time and shake off better.  Symptoms of BPH  seem a little worse over last couple years.  I’m hoping some type of urine back flow infection. I am scheduled to see my doctor Monday.  But I’m sure you are more knowledgeable about STD.     Sad to say I’m married and no excuses for drinking too much.

Update just received paperwork and both tests were IgG .  Hsv1 was 15.0 and Hsv2 was 6.62.

So both indicate solidly that you are infected with both viruses.  I believe that you have HSV 1 orally (cold sores) and HSV 2 genitally.  And I don't believe that you acquired HSV 2 from receiving oral sex once or having intercourse with a condom - is it possible?  yes.  It is likely?  NO
Has your wife been tested to determine her status?

Thank you for information.    I have managed to stay away from my wife as far as sex.   My wife don’t  don’t deserve this and I know I am going to have to tell her.   Very difficult and emotionally stressful!     I ordered book from your web site  Managinig Herpes and I will face the consequences of my actions.  Last thing I want to do is hurt my wife .    Lastly about  14 days after encounter,  itching in  genital area started.  I paid well known national clinic for physical exam.    They said no signs of hsv2 . Then almost  4 weeks later after initial start of itching I still had slight itch and saw my own doctor.   Doctor also said no signs of hsv2.  I had told them both about Quest lab  blood test being positive.  My own daily self exam I never saw any signs. .   Both professionals exam revealed I do not have hsv2 and both said it was psychological.   I suspected I had hsv2 and I concur with you.   We will  use medical insurance to see some one with expertise in field.  Should we see a dermatologist?   And as far my itching not be diagnosed by two professionals exams is  itching only considered my symptoms/my “first “ outbreak?  ( Maybe I never developed  any lesions because I had always had hsv2?). I guess because of guilt now it’s psychological because on occasion I feel a slight itch.

you could have had herpes for years - even before you met your wife if you had sex with other people before her.  Or she could have infected her.  No way to know unless she gets tested.  A person cannot tell if you have herpes by a physical exam unless you have a lesion present at the time of the visit.  And to say you are uninfected with HSV 2 with an index value of 6.6 is malpractice plain and simple, if they knew that fact.  Here is what the CDC in their 2015 Sexually Transmitted Treatment Guidelines say about a positive HSV 2 serology test:

"Because nearly all HSV-2 infections are sexually acquired, the presence of type-specific HSV-2 antibody implies anogenital infection. In this instance, education and counseling appropriate for persons with genital HSV infections should be provided. "

It would be good for your wife to be tested but I feel very strongly that you did not get HSV 2 from the encounter that you describe.  I don't know how much detail is necessary about that for the discussion with your wife.
