[Question #4769] #4767 Follow up post GUM Visit & Diagnosis

Hello Doctors,

This new question is a follow up to my conversation yesterday with Dr. Hook. I visited the GUM clinic today, had a swab Test and was diagnosed with NSU (between 5 and 20 WBC found). Urine samples have been sent away for analysis for Chlamydia, ghonnoreah and MG. I have been prescribed 1 week of Doxcycline.

I have informed the girl from 2 weeks ago that I had unprotected and she has an appnt on Monday to be treated.

I have also informed my wife of the NsU diagnosis and she has been prescribed 2gs of Azithromycin as she is allergic to Doxycycline.

Questions
1. I didn't mention this yesterday but on 1st January (3 days ago) I also had condom protected sex with the girl I had unprotected sex with two weeks ago, and she performed unprotected fellatio on me and me unprotected cunnilnugus on her.

If I start taking the Doxy tomorrow - 4 days post this most recent (and final!) encounter with her will this be enough time to eradicate any new infection which may have been picked up from her from this most recent encounter? My concern here is that I may have got NSU from her from the first encounter a couple of weeks ago which I now treat with the Doxy but then perhaps a new infection appears which I then have to test for again in a couple of weeks.

In short, will the doxy clear any infection from the first encounter two weeks ago and the most recent encounter 3 days ago? (Assuming ghonnoreah, Chlamydia and Mg come back negative of course).

2. Is the 2gs Azithromycin effective for my wife given she's allergic to Doxy?

Thanks Doctor

Dr. Hook warned me that you'd probably be back with more anxiety driven questions. You're really obsessed, quite irrationally, with NGU (NSU) -- going back at least a year and a half, when you posted two questions under a different email address and forum registration. And since you're under expert care at a GUM clinic, that's a more appropriate source for these questions than a distant online forum. I'm also rather amazed to learn that despite your fears, you went ahead with yet another potentially high risk exposure that you "forgot" to mention previously. I hope you understand the psychological implications of the combination of these fears and your behaviors. Very brief replies, then immediate closure of this thread.

1) Why the 4 day delay in starting doxycycline? The sooner the better to eradicate any existing infection or to one to which you might have been exposed.

2) 2 g azithro is double the usual dose. Elevating the dose from 1g to 2 g is to cover gonorrhea in addition to chlamydia. In any case, it will be more than adequate in case she has or has been exposed to the main causes of NGU.

As implied above, it is not appropriate to use this forum for clinical information when already in care by experts as equally qualified as we are. And please note the forum does not permit repeated questions on the same topic or exposure. This will have to be your last one; future new questions about similar exposure and/or your concerns about NGU may receive no reply and the posting fee will not be refunded. This policy is based on compassion, not criticism, and to reduce temptations to keep paying for questions with obvious answers. In addition, experience shows that continued answers tend to prolong users' anxieties rather than reducing them. Finally, such questions have little educational value for other users, one of the forum's main purposes. Thanks for your understanding.
