[Question #4784] Tingling/Burning on penis head, buttocks, groin, and behind (inner) thighs.

Hello Doctor, I've read through multiple posts on here and would like some reassurance and expertise on my specific problem. I went to a Strip Club on Christmas Evening and had 'protected' sex with a stripper. During intercourse my penis was becoming flaccid and we stopped, upon which she began to masturbate me (with the condom on) in an attempt to make me hard. She also rubbed my scrotum 'after' masturbating my penis. It didn't occur to me till later that vaginal fluid could have been on the condom when she masturbated me and then rubbed my exposed testicles since I had already been inside her (I also used lube). Would this be a risk of acquiring Herpes? Also, I was sitting naked at one point on a leather type of sofa and am wondering if anything is transmitted that way since my bare butt and inner parts we exposed and now they're itching and burning.   FYI, I've experienced this tingling/burning on my groin area before about a year ago or less and have got tested and everything came back NEGATIVE except for HSV-1, which was a blood test. When I experience these initial symptoms  I went to the doctor about 3 different times within a couple of weeks (when i was experiencing symptoms) and they all said that it did not look like herpes since I had NO lesions, sores, or cuts. They all said it was jock itch or 'Ballanitis" and gave me some ointment. So it's been 1 week since my last sexual encounter and it was PROTECTED but want to know if: 1.) could I have contracted HSV-2 via her rubbing my testicles when she masturbated me with a condom on AFTER I was already inside her beforehand? I'm wondering if the rubbing could have transmitted it. 2.) Does my testicles slapping up against her vaginal area make it a risk for catching Herpes? Not sure if they touched her.3.) What about my burning pain on my buttocks/inner thighs? Could I have caught anything from the sofa?? Keep in mind other strippers use the same sofa every day and who knows who else has has sex in there.

Hi. I would like to ask a 2nd question since I originally asked 1 but created a 2nd post when I thought my question was overlooked. This is regarding the same incident. I got tested 19 days post exposure for both HSV1 and 2 (IGG) blood tests and only HSV1 came back positive with a "high" count (>1.09 is positive). Obviously, this is too early to detect for HSV2 so I am waiting. However, I've been experiencing symptoms since and even have the leg burning, aches, and feeling of tiny cuts on my genitals. My original question was if I could have contracted HSV2 through a female massaging my scrotum with her hand containing vaginal secretions. Vaginal sex was protected. You said not very likely at all and I've also read multiple posts from the other Doctors who say mutual masturbation is very low risk. However, I think I may have contracted HSV2. I have been taking pictures since I started noticing symptoms and after careful review noticed that under my scrotum there were what appeared to be "bi lateral" red bumps (primary infection, no?) They appeared about 10-12 days post exposure. I went to urgent care and planned parenthood about 4 times but they all said it did not look like herpes but maybe i went too late. One NP said it looked like a genital wart but other NPs said it's not a genital wart, but more like folliculitis. 

NOW, 1 week ago i noticed a couple of "tiny sore/cuts" on my penis head below the head where the foreskin retracts and I was inspecting them with my fingers. There was also some redness/irritation. I forgot to wash my hands and about 45 mins later I rubbed my eyes/slight scratching and realized what I had done.  Since about 5 days my eyes have been irritated and I have been experiencing teary eyes, dryness, feeling of something inside my eyes. I'm afraid I spread herpes into my eyes. My skin is irritated and I feel pressure and I noticed a tiny cut below my eye where the lining is. I'm very afraid I might have done something horrible and will get Ocular Herpes. I went to the Optometrist twice already and he performed the slit test to check for herpes but nothing yet. He did say my eyes were very irritated and had some concern. Also, he did notice the tiny cut but didn't think it was herpes but in his words, he's not an MD. From all the rubbing I've been doing I may have spread the virus on to other parts of my face as I can see some small lesions on my forehead and cheek. I know they're not normal since I'm experiencing burning sensations on my face.  One of the lesions seems like it crusted and it seemed "yellowish."

I'm very scared. When I initially tested positive for HSV1 they gave a prescription of Acyclovir 400MG but I haven't taken it since I don't want it to affect my blood test. 

1.) Do you think I should begin taking the antivirals?? I'm fearful for my eyesight. 

2.) If I did get herpes, could it be HSV1 or HSV2? I suspect I already had HSV1 genitally prior to the last sexual exposure I described and so am afraid I acquired HSV2 and spread it to my eyes/face. 

I have been so depressed for a month now with enormous amount of anxiety so maybe this has made my immune system weaker. I just hope it's not HSV2 on my face or scrotum.

3.) Should I begin taking the antivirals?? I'm been putting them off for the sake of getting an accurate blood test and also building immunity to HSV2 in case I did indeed get it.

I understand that you are very scared.  That's an awful feeling when it comes to an STI
1.  NO.  If you want to take another antibody test later, the antivirals will influence your test results and could result in a false negative.
2.  Your encounter Presented such little risk that I think your worries are considerably overblown.  The likelihood that you would acquire herpes from someone massaging your scrotum if they had vaginal secretions on their hand is about as close to zero as you can get. I have no idea why you think you have it just a one genital infection  but in regards to the this encounter and HSV-2, I would not be at all concerned in your situation. Which means that it is highly unlikely that you would've transmitted it to your eye. I think you have taken this worry and just run with it and that's not reasonable in this situation nor is it good for your mental health. My suggestion to you would be to wait 12 weeks from the encounter and if you wish, do another IgG test. Or, you could skip the IgG and go directly to the herpes of Western blot antibody test which is considered the gold standard diagnostic method for this disease. Seeing how worried you are, I think it might be difficult for you to accept the results of the IgG test if it is negative, because it does miss some cases of HSV-2 infection.

If I did indeed transmit HSV2 to my eyes or area around my eyes/face, how bad can it be?? I've read that HSV2 (oral) tends to not recur but I'm not sure how this would affect my eyes/facial area. I truly feel something is going on on my face and i see some very subtle lesions that are already dry/drying. Since exposure to my eyes, they were very irritated and hence I spread the irritation across my face. If this was indeed a first outbreak on my eyes/face, this would be the worse one, correct? I can't believe how stupid I was to forget to wash my hands. I'm fearful for my eyesight and overall facial appearance.

Also, I haven't shaved because I'm afraid of shaving over some potential lesions and causing an infection and spreading them more. Is this a concern i should have?

Ocular herpes can cause blindness so it can be severe.  But most often, if people get herpes of the eye, daily antiviral medicine will prevent blindness.  I think you are worrying too much about this possibility and you can also shave.  If you have HSV 1 facially, it is all of those nerves or can travel to all of those nerves, without you doing anything.  So this should not be a concern.
