[Question #4814] STI risk from massage

Hi Drs.,
Around 4-5 days ago, I received a massage where I was naked, and the masseuse massaged my entire body. She also massaged my buttocks with her bare hands for several minutes. After I came back, I observed I was bleeding a little and there was exposed cut skin near my tailbone area. I usually get pilonidal cyst there. so I assumed it had cut open during the massage. During the massage, she did not directly touch or rub my penis , but her hands were rubbing against my scrotum while she was massaging me. I started getting sore left buttock and also pain in my left foot from next day. Today I am feeling lower back pain and pain in my neck on left side too. My scrotum is also itching a bit today. Does the cut skin near my buttocks and other activities put me at risk for any STI or herpes? Do I need any testing?

Some questions can be accurately answered from only the information in the title. Before I even read the question itself, I can say that STIs are rarely if ever transmitted by massage, assuming no actual sex (e.g. oral sex). This includes massage with "happy ending" by hand-genital contact.
Now I have read the question. Guess what? You describe nothing that makes me suspicious that you were at risk for any STI or that any such infection is the cause or your symptoms. I really wouldn't worry about it and do not recommend any testing. If you have a regular sexual partner, you can safely continue your normal sexual activities without risk of trnasmitting anything related to your massage.

I had a red bump- small pustule filled with some fluid with red base show up on my lower back..around couple of inches above my tailbone around 5 days after the massage. The RN I saw said it looked like a superficial skin infection to her and prescribed anti bacterial cream. Another similar bump popped up next to the first one the next day. Then I had a small bump on my left inner thigh. I am worried if this could be HSV or another infection from the massage place. since I have been feeling pain in my legs and generally run down past few days. She did not massage my genitals but my broken pilonidal cyst is causing concern.

Herpes doesn't behave like this. Initial HSV infections cause blister-like lesions only at the place(s) directly exposed; and the lesions appear together in a relatively restricted area, not as widespread single ones like you describe. And initial herpes infections also don't cause pain in the legs. There is no chance this is herpes, or any other infection from the exposure you are concerned about -- and as I said, no STDs are transmitted by massage, including herpes. Finally, if your pilonidal sinus were exposed to herpes, that's exactly where the main symptoms would be occurring.
Do your best to move on wihtout further worry. There was no STD risk.

Thanks Dr. HHH. I saw an Urgent Care who confirmed I had active pilonidal cyst and referred me to dermatology since it has been an ongoing problem for me. Since I have the opportunity, would like to ask your expert opinion. My sexual history summary:
1. Two times protected intercourse with unknown women (last one was 1 year ago)
2. 3-4 times protected oral sex (last one more than 1 year ago) and 3-4 unprotected handjobs from unknown women (last one more than 4 months ago)
3. Regular unprotected sex with my ex-GF (last one more than 5 years ago, she was my first and I was her second sexual partner at the time)
My ex-GF was diagnosed with high risk HPV around 1.5 years ago (she had other partners after we broke up). Since there is no test for men, what is the probability of me having an active high risk or low risk strain of HPV ? I ask this, since I am planning to get into long term relationship soon (possible marriage) this year, so want to know if I should ask my current GF to take HPV vaccine ? I come from country where STI sigma is high, and HPV vaccines are still not administered as a policy. (last time they tried it in schools there was opposition from some fanatical groups).

Also, I have tested negative for all common STIs (including HSV IGG and WB) since my last protected intercourse more than an year ago. Only exposure after that has been one unprotected handjob. Can I be confident I don't have any of those STIs? Is there need for getting tested for STIs in the mouth or throat, since I did kiss breasts of unknown women couple times (never gave oral sex though) ?

Sorry Dr. H, last question: since the day I received the massage almost 4 weeks ago, I have had a sore left buttock, pains which I feel when I sit most of the time, also some lower back pain. The sore buttock started almost immediately after the massage, so it can't be any infection related right ? Mostly likely some trauma from the actual massage?

I'm of course not surprised that the pilonidal cyst was confirmed, not herpes or any other STD. As for your buttock pain, no infection can start to cause symptoms of any kind for at least a day or two. With immediate onset of discomfort, it obvously had to be from the direct trauma from the massage, or perhaps an emotional reaction to the experience -- but not possibly any infection from that event.
