[Question #4816] Unprotected Vaginal Sex

Me 37m. had On 22/12/18 only once unprotected vaginal sex with a lady in indian massage parlour for 3-4 mins. I was on top. She looked clean and healthy. not aware of her hiv status. after 2 hrs i met a sexologist doctor but he gave me medicine for STDs only. No PEP was prescribed. As each passing day is making me more tensed and worried, i took hiv 4th gen combo test on 6th Jan 2019. result negative and non reactive. and again hiv combo and RNA PCR Quantative on 11th jan 2019. Combo again negative and non reative , PCR RNA target not detected. Can it be a good sign and can i relax. I only have mild sore throat from 7th jan night and very little sores. no other symptom so far. So please suggest where do i stand in terms of HIV fear. Thanks and Regards

Welcome to our Forum. I'll be glad to comment. This risk for acquiring HIV from the exposure you describe is very low. The majority of commercial sex workers do not have HIV and even if your partner did have untreated HIV, the risk for acquisition of infection from a single exposure of the sort you describe is less than 1 infection in 1,200 encounters(in other words, 99.9% of exposures will not lead to infection). Your negative tests make it far more unlikely that you acquired HIV. At three weeks after exposure (your most recent test) combination HIV antigen/antibody tests would detect about 90% of recent infections and still more would be detected by the HIV RNA test. This I would now estimate that your risk for infection is less than 1 in several million, at most and personally would have no further concerns what so ever. If you wish to be absolutely sure that you were not infected, my advice would be to test again with the combination test 6 weeks after the exposure (on February 2 or beyond). At that time results will be entirely conclusive. Should you choose to test again (personally, as I said above, I would not bother), I am confident that the test will be negative, proving that you were not infected.

Sir, after unprotected vaginal sex on 22nd Dec 18, I have following test:
06/Jan/19 HIV 1&2 and p24 combo test. Results negative and non reactive
11/Jan/19 HIV 1&2 and p24 combo test. Results negative and non reactive
11/Jan/19 HIV RNA PCR Quantitative. Results on 13/Jan/19 Target not detected.
19/Jan/19 HIV 1&2 and p24 combo test. Results negative and non reactive.
Please suggest if these results are conclusive. Can I relax now and can I have unprotected sex with my wife again.

Please re-read my answer above. It does not change. The additional test result you mention is a week closer to being completely conclusive but does not change the fact that the official recommendation that your results will not be entirely conclusive until February 2 or later. As I also explained earlier, the result that you do have make it very close to certain that you were not infected and in my opinion, your reults are so close to being entirely conclusive that there is no practical value in further testing. Hello doctor, today I got test at 6weeks by HIV combo 4th gen test. Result is negative and non reactive. Is it conclusive? Am I hiv free for sure? Or any more test is required? Kindly suggest.

I'm surprised that you felt the need to ask this question again since it was answered earlier. As I told you earlier, for all practical purposes you had conclusive results 13 days ago when I last responded, now they are "official". Thus, the answer has not changed- you have proven that you were not infected with HIV. There is no need for further testing. To do so would be a waste of your time and money.
This completes this thread which will be closed shortly without further answers. I hope that you will be able to move forward without continuing concerns. EWH
