Had unprotected oral with male stranger he say he is negative and didnt have sex since it lasted 30sec no ejaculation i had a pcr rna and antibody test at 23 days

You don't say the "direction" of the oral sex event:  your penis in his mouth or vice versa? But it probably doesn't matter much. Oral sex is safe sex in regard to HIV. There has never been a proved case of HIV transmission mouth to penis, and only rare cases penis to mouth. One calculatio suggests the average risk is 1 in 20,000 mourh ro pwnia nS 1 IN 10,000 in the other direction. That's equivalent to receiving BJs by infected partners once daily for 55 years before infection migh be likely; or giving BJs to infected men once daily for 27 years. In other words, zero risk for all practical purposes.

In addition, I agree with your doctor. The combination of negative HIV PCR and antibody at 23 days is nearly 100% conclusive. For truly 100% conclusive results, have an antigen-antibody (duo) test at 5 weeks. But in the meantime, no worries:  there is no realixtic chance you have HIV.

Just wondering what you mean by no risk for practical reason and why is the conclusive is 5 week instead of 4 weeks

"No realistic chance you have HIV" means that the risk isn't zero, but is it very unlikely. Even if you had not have the negative blood tests, I would put the chance you caught HIV at well under 1 chance in a million.

For the most commonly used HIV blood test, the antigen-antibody (AgAb test, also called "duo" or "combo" test), it takes 6 weeks for conclusive reesults. Having had probably that test PLUS the PCR test at 4 weeks, the chance you have HIV is under 1 in a billion. That's zero for all practical purposes, which probably is why your doctor said the results are 100% reliable.

If you remain concerned or uncertain, you could have another AgAb test 6 weeks after the exposure. Or discuss it again with your doctor:  I'll bet he will agree with my comments. (You could print out this thread as a framework for discussing it.)

Thats pretty much what he said except the 6 week 4th gen test which i consider just to put my mind fully at ease
Zero for all practicle purpose mean that it would be a extraordinary case thats plausible but almost impossible, just to fully understand

As I said above, I would estimate the chance you have HIV as 1 in a billion. I’m sure you’ll agree that’s “almost impossible”. It’s less than the chance you’ll be struck by a meteorite in the coming year.
