Hello Dr. Handsfield. I appreciate your reply regarding the 4 rapid tests (3rd and 4th gen finger prick) and whether I could rely on the results. At the time, it provided me with assurance that I could rely on the test results. I'd like to say I've moved on, but am now having trouble again. I began to see a psychiatrist for anxiety and stress. I've been on Sertraline (100mg) for about 4 weeks. Can't really say that it's helping. The pain in my legs went away, only to resurface with warm sensations in both feet that come and go. Have them now as I'm writing this. Also went to a neurologist and he didn't think it was neuropathy. He sent me for more blood work (CBC, Metabolic Panel, Vitamin B6 and B12). All came back normal but Vitamin B6 was high due to me taking a B6 supplement for the nerves and sensation in my feet. Not diabetes since blood sugar was normal. I also have other symptoms that have surfaced and am getting nervous that my test results are flawed or I didn't wait long enough. When I started on the Vitamin B6, I noticed my urine had a pungent (not foul) smell. I stopped the vitamin 2 wks ago and still have the odor. Not sure why? More concerning is that I noticed enlarged taste buds on the back of my tongue 2 wks ago also. Of course I Googled it and it looks like its enlarged Circumvallate Papillae. Possibly from excessive mouthwash use which contained alcohol, or is it a sign of HIV? So I went to an ENT Doctor about an hour ago. He checked my tongue and told me to use baking soda and rinse with kosher salt. He also said there was a "coating" but nothing to worry about. When I saw the paper he gave me it says on the paper hairy leukoplakia. I said to him I thought that's a symptom of HIV. Again, he said people without HIV get this and you were already tested so not to worry. Please reassure me I don't need to go for more HIV testing. Very concerned and not sure why this is happening.

Welcome back to the Forum. On this occasion, I will be answering your follow-up question. Dr. Handsfield and I share the Forum and today I happened to pick up your question. I did review your interaction with Dr. Handsfield a month ago however and agree with all that he said, your testing has proved, again and again, that you did not acquire HIV from your encounter of concern.
Reading the description of your activities and healthcare encounters since you interacted with Dr. Handsfield, I can say that it sounds like the specialists you saw (the neurologist and ENT) have done a good job of evaluating you. From the sounds of things you are in a state of "hyper awareness" which is causing you to look at yourself (in your mouth, the smell of your urine, etc.) which is causing you to see things that were likely present in the past but unnoticed. This is not uncommon among our clients who are worried that they might have been exposed to an STI during an encounter which they subsequently wished they had not had. Working with a psychiatrist is the right way to approach this problem- does he/she know that you have seen the neurologist and ENT? If not , I suggest that you do.
My most important advice however is to continue on your efforts not to worry about anSTI from the encounter you described. You did nto get HIV- that is certain and no additional testing is needed. EWH

Thank you Dr. Hook for your prompt reply. It's a wonderful service you provide to your clients. As to your question, I just came home from a session with my psychiatrist and told him I saw the ENT and Neurologist. He is trying hard to work with me to get past this. All doctors I've been to keep telling me the tests I had (even though Rapid Tests and not lab based) are conclusive at the times I took them, but my concern is the enlarged papillae on back of tongue and diagnosis from the ENT that said "hairy leukoplakia"? When I google that, is says sign of HIV. My tongue doesn't look like the pictures I see on the internet. Is there a chance the tests were wrong or I didn't wait long enough? Or perhaps I need to have a lab based test done to be sure and get past this? My apologies. I thought I was moving on until today's visit with the ENT. He didn't seem concerned so why am I?

Thanks for your thanks. I am concerned that, understandingly, you are trying to find answers on the Internet and are being misled. Unfortunately, going to the Internet is more likely to mislead anxious persons than help. Much of what is there is taken out of context, out of date, or just plain wrong.
You absolutely do not need more testing for HIV of any sort. Your results are completely and incontravertably conclusive. Do not let the Internet mislead you. EWH

Thank you for your last reply Dr Hook. As you've seen from my previous posts, I've taken 4 Rapid Tests - 2 (3rd gen) and 2( 4th gen), all beyond 10 weeks. Each time I took the test left feeling reassured I didn't have HIV, yet days or weeks later the feeling creeps back in that the tests were either wrong, I didn't wait long enough, my antibodies were delayed, etc. I realize that both yourself and Dr. Handsfield say conclusively that these rapid test results prove I don't have HIV. I just have two remaining questions and will then hopefully be able to put this past me.
1) I've seen 2 ENT's over the past two days for the enlarged circumvallate papillae on the back of my tongue. The first doctor I saw was unfortunately quite rude. He mentioned my tongue had a "coating" and I should rinse with salt water and use baking soda. I just don't understand why he would write down "hairy leukoplakia"?? I look online and my tongue looks nothing like what I see on the internet. It freaks me out because the condition he wrote down is associated with HIV. So yet again, if the previous tests I took are reliable should I be concerned? The second ENT I saw yesterday was excellent. He too said he saw a coating on my tongue but said nothing about leukoplakia. I told him what the first ENT said and he said no. That typically shows up on the sides of tongue and cheeks. Not sure what to believe. He took a culture which I'll have back in one week. He said the coated tongue comes from dead bacteria cells and that I should add more roughage into my diet. Obviously the first diagnosis is causing me to re-think the validity of the tests I had.
2) Based on the above, is there any reason at all I should go for another test? Are the tests I took as reliable as a lab based test or should I go to the lab?
I realize this is my last question and truly appreciate the great work and responsiveness you and Dr. Handsfield provide to your clients day in and day out. Take care of yourself.

Please don't let an ill-advised comment by the 1rst ENT you spoke with upset of shake the confidence that you should have in your test results. Answers to your questions below:
1. See my comment above. Ignore the comments of the 1rst ENT.. Your tests are TOTALLY reliable and should be believed. Parenthetically I should also point out that while there is an association of hairy leukoplakia with HIV, the fact is, most people with HL do NOT have HIV. Further, you do not have HL.
2. No, to scientific or medical reason for further testing related to your exposure of concern.
