Hello docs,
Briefly, i had one risk situation with a prostitute, first time i used condom and finally i didnt finished (i was drunk) but when i was out i saw that condom was slipped off half of my penis (the head and uretra was covered).I observed that in part of penis uncovered was a huge quantitty of vaginal fluid.After 1 minute around i put another condom without cleaning the liquid from there and the prostitute give me one handjob until i finished.My question is wich is my risk, the virus was there alive and inside the condom maybe go inside of my uretra or foreskin? Can survive the virus outside the body?

Welcome to the forum. THanks for your question.
First, although many sex workers are at high risk of HIV, among even those at highest risk, under 1% have HIV. In addition, even with entirely unprotected sex, the averqge chance of HIV transmission for the male is about 1 in 2,500, if the female partner is infected. Second, your question suggests you know that condoms prevent HIV primarily by covering the urethra and the head of the penis. Contact of the skin of the rest of your penis with vaginal fluids carries little if any risk. And hand-genital contact never transmits HIV, even when genital fluids are used for lubrication. Whether or how long the virus survives outside the body really doesn't matter. However long it is, HIV is not transmitted by such contact.
All things considered, I cannot say the risk of HIV from this event was zero, but it was very low, probably under one chance in a miliion. You probably will want to be tested for HIV in a few weeks, but you should not be seriously worried in the meantime. It is extremely unlikely you caught HIV. The risk of other STDs also was low, although probably somewhat higher than for HIV. You can be accurately tested for gonorrhea and chlamydia (with a urine or urethral swab) after 3-4 days, and for syphilis (along with HIV) at about 6 weeks.

Hello,
I asked how much time survive hiv outside the body because if it not dead before to wear second cundom the vaginal fluid which was at shaft can reach in foreskin and uretra by handjob,by friction and also inside the custom is good medium same like inside the vagina.Also i am a little bit concern because the risk was in Brasil,where percentage i think is higher.What about hiv 2,also at 6 weeks it is definitive or have to test later.Which is situation in Brasil,are many cases of hiv 2?I asked about hiv 2 because this dont have p24,i think and if my body dont produce antibodies maybe have to wait more time?

As I said, there have been no reported transmissions of HIV because of contact with HIV in the environment, by hand-genital contact, etc. Therefore, how long HIV survives is not important.
I think you have the impression or belief that any exposure to HIV could result in transmission, that "just one virus" would be enough if it entered the body. This is not the case for HIV. It takes lots of virus, which has to be exposed to certain cells that for the most part are deep inside the body, for infection to take hold. Exposures to small amount of virus is not risky, perhaps with very rare exceptions. Look at it this way: if an infected man deposits his semen, loaded with HIV, deep into an uninfected woman's vagina, there is only one chance in 1,000 she will be infected. (Many wives of infected men never catch HIV even after months or years of unprotected sex.) With such a low chance from this kind of exposure, what can the risk be from minor exposures to small amounts of virus?
The modern (3rd and 4th generation) HIV tests are considered conclusive for HIV2 at 6 weeks, although some experts recommend 3 months to provide an extra margin of safety.
There is no such thing as HIV infected people whose bodies "don't produce antibodies". It never happens. That's one reason the tests are always 100% reliable.

Good day,
Today i received the results arn test hiv 1 and Architect Combi test,both negative at 3 months from risk.All this time beginning with 3rd day untill now i had almost all symptoms (i still have body pain,throat pain,loss of weight).Now i am almost sure hiv 1 it is ok,my concern remain hiv2, because my risk was in Brasil and there i think are many cases.Finally doc i have these question,i hope after that to close this chapter of my life and move on.
1.Can i consider these 2 tests at 3 months final or have to test again at 6 months for hiv 2,my worries are these symptoms which still i experience.In my country almost all docs ask to test at 6 months.
2.You saw in last decade one test at 3 months negative and after that to become positive.
Give me one advise to move on,still i have one strong ,,feeling’’’ that i am positive.

1) Your results already are conclusive for HIV2. No additional testing is needed.
2) Delayed test results used to occur rarely with the early HIV antibody tests. It never occurs with the current tests, including the combi tests you have had.
