[Question #4989] HIV RISK FOR ORAL SEX

Dear Dr,
I am from Malaysia. I am married guy. I had french kiss with 1 girl on 28th Sep 17 which unknown HIV status. She had always lips skin crack and i always had some blood when i brush teeth. The duration is around 2 minute. I am freaked out and ask her to check HIV & STD on 4th Oct 17 and the status is negative.
On 13th Oct 17, she perform a blow job to me and i eject to her mouth without protection. I am not sure she had any cut or sores in her mouth and i also not sure my penis status. No other sex activity anymore with her. She declare she are clear but i know she active in sex. She go to test HIV antibody again in private hospital on 28th Oct 17, 17th Nov & 21st Dec and the result is negative.
I had also go to test on 12th Mar 18 in private hospital and the result is negative. Unfortunate, we do not know what type of generation and brand of the hospital use. after that, i thought the result is conclusive and i didn't met the girl anymore.
i had a long time didn't touch my wife as i forbia.
Me continue sex with my wife recently without protection, but after that she get the high fever 11 days and unable to recovery until now. All the blood test & MRI is normal and no virus infection. doctor also say that is weird and suspect antibody continue fighting even no virus detection. All the test done except HIV & STD.
My question as below,
1. Is It I should go to test HIV 1 more time? Or you think my last HIV antibody result 100% conclusive?
2. Do you think no matter in what situation, the oral sex without protection is safe from HIV?
3. Do you think am i affected my wife with HIV?

Welcome to the Forum. I'll be glad to comment. The short answer to your questions is that there is no risk at all from French kissing or from receipt of oral sex in the unlikely circumstance that your partner happened to have HIV (most people do not have HIV). This is true even if your partner had blood in or open sores in their mouth or, related to kissing, if you had bleeding lesions or open sores in your mouth. No one has EVER been proven to have gotten HIV from French kissing or from receipt of oral sex and you are not going to be the first. Your test results prove that you were not infected. No matter what the "generation" of test, test results obtained more than 12 weeks after exposure have always been conclusive. Thus in answer to your specific questions:
1. Is It I should go to test HIV 1 more time? Or you think my last HIV antibody result 100% conclusive?
You results are entirely conclusive. There is no reason for additional testing.
2. Do you think no matter in what situation, the oral sex without protection is safe from HIV?
Yes
3. Do you think am i affected my wife with HIV?
Your wife's recent symptoms are coincidental and not related to the exposures you have describe.

Thanks for your reply.
Based on by understanding even the girl had HIV and perform the oral sex to me(my penis to her mouth), i still consider safe even open sore for both party? I am worry because some people in other web say that they get HIV from oral sex.
In addition, the malaysia doctor told me that i should check after 6 month and 1 year exposure, because some hiv will only appear at that time. Therefore i am worry for that as i not dare to retest again.

You have repeated what I said above correctly. There is no proven risk for HIV from receipt of oral sex. Persons have clearly acquired HIV after performing oral sex on an infected partner but this too is a VERY rare event, occurring less than once in every 10,000 exposures. As I said above, your exposure was a no risk event.
Your Malaysian doctor is being overly conservative. My recommendations have not changed.
Finally, I urge you to stay off of the internet. As we have told many others, much of what is on the internet is incorrect, either because if it out of date, taken out of context or just plain wrong. EWH

Dear Dr,
Last question for me that if i didnt do any test for after this single episode for french kiss and oral sex only, will you recommend me go to do hiv test if my wife had this sysmtons of ars?

Based on the information you have provided, I see no need for testing for HIV related to those events. If you feel you must test for your own peace of mind (I understand that you might want to do this), then I would wait until six weeks after your last exposure to test. At that time results will be entirely conclusive.
In addition:
1. for all practical purposes, you cannot transmit infection to your wife if your own HIV test is negative.
2. More than 99% of persons who have ARS symptoms do NOT have HIV but instead have other non-STI flu-like illnesses.
3. If your wife has ARS symptoms, and your HIV test is negative, her symptoms are not due to HIV that you gave her.
