[Question #5056] HSV

Hi I’m a 43 year old Female , had first of many encounters on 1/26/19 had unprotected oral and vaginal sexual 
intercourse with a known partner. Had sex with that same partner the previous year and tested negative for stds afterwards.
2/16/19 Fever small painful blisters around labia , vaginal 
opening. Painful urination 
2/17/19 Saw Urgent care doctor told genital herpes! 
Put on 1 gram Valtrex twice daily , helped a lot sores gone by that Friday 2/22/19
2/25/19 Saw regular Gyn. He was unable to see any sores 
Did HSV bloodwork 
HSV 1& 2 Spec AB w/Rfx 
IGG HSV 1 POSITIVE at 1.11 
IGG Negative for HSV2
HSV 1& 2 IGM POSITIVE at 1.64 
My Gynecologist says I have HSV1??? 
Partner had canker sore inside mouth when I received oral.  What are the chances that I have BOTH HSV1 being an old infection and HSV2 being recently infected?
Also I have one other partner that I have been with for 3 years on and off and he will do bloodwork to test for HSV but I haven’t resumed sexual relations with anyone since finding out I have genital herpes.

Completely ignore the IgM test - it has, in my experience, about 85-90% false positives and the CDC says never to use it to try to diagnose herpes.  It is a shame it was done at all. 
Your symptoms do sound like herpes BUT if you have been taking daily antiviral treatment since your visual diagnosis, it will most definitely impact the IgG test.  Was no swab testing done of the sores? 
Has your partner been IgG tested for HSV?  If not, he should be and I'm glad the other person is doing it.
You may wish to consider going off the antiviral medicine and waiting a few months and doing the testing again.
Have you ever in your life had a cold sore on your lip?
Do NOT assume you have HSV 1 genitally at this point - not enough information for that.

Hi thank you for answering my question. That particular partner hasn’t been tested yet... I have been requesting that he please get tested ASAP. 
I am no longer on Valtrex. I only took anti virals from 2/17-2/25
(Blood tests were done on the 25th )
On 2/17 When I went to Urgent Care , I didn’t have insurance, was in Hawaii at the time, they didn’t swab the sores 
And on 2/25 when I came back to Maryland my regular Gynecologist didn’t see any sores outside the vagina or inside. They healed completely in a week with no crusting or anything.

I haven’t had any cold sores that I remember maybe a tiny painful cut on the outer corner of mouth.
5 years ago when I was married I saw a urologist who claimed the small cut on my  outer labia lips was a sore and put me on Valtrex.  My ex husband always had canker sores inside mouth. 
She did an IGG which was negative so I was told probably not herpes. I haven’t had sores only feels like cuts around perineum after sexual intercourse  and a burning feeling during sex sometimes. I also have burning inside Urethra sometimes but I told maybe Interstitial Cystitis. 
However this particular outbreak  was  like the classic herpes with fever , chills and very painful blisters. 
Do you think maybe I have Type2 genitally and also type1 orally? 
Should I get retested in 3 months time?

Well, you did have something and the trick is to find out what.
Yes, I would suggest that you repeat the test when 12 weeks have passed since the encounter (you could test at 6 weeks with 70% sensitivity on the test).  The western blot is the best test but a positive IgG result at greater than 3.5 would be diagnostic.  The IgG test misses 30% of HSV 1 infections and 8% of HSV 2 infections compared to the western blot

Ok thank you. I’ll try to get the guy tested as well . If not , I will wait to test again at 12 weeks and if I have another outbreak I will go get it cultured.
