[Question #5229] Brief Oral Sex No Ejaculation

Hi - I am a male and sucked another male of unknown status (unprotected) for around 30 seconds or less with no ejaculation.  I do have healthy gums and mouth.  I did have a split lip that was pretty much healed (no blood or anything).  As this is my only exposure, do I need to worry about this?  My understanding all oral HIV cases happened with ejaculation and the fact there was no ejaculation and it was so brief, I am assuming worrying will be for nothing.  If I choose to do an RNA test (to put behind me) should I wait 10 or 12 days; the literature says 9 to 11 days.  Or is that a waste of time/money and I shouldnt worry about having sex with my significant other (female).

Welcome to the Forum.  I'll be glad to comment.  Performance of oral sex on a male is among the lowest risk of all sexual activities.  Transmission of HIV in this setting is so low it is quite hard to study so when it comes to the effect of ejaculation there are no good answers.  HIV is certainly present in pre-ejaculate as well as ejaculate but at least theoretically, if your partner was infected, the fact that he did not ejaculate should be associated with a lower risk for infection.  If you choose to do an RNA PCR test at 11 days or more, that would be positive before the standard combination HIV antigen/antibody test

So - should I test?  If so. And I test negative at 11 days for rna, should I move on with life.  I don’t want to test if I shouldn’t.  I read there needs to be a perfect storm for oral transmission - ejaculate, bad gums, meth mouth, etc...  I guess I was hoping with it being so brief and none of those factors that I wouldn’t be at risk.

This depends on your level of concern.  The only way to be absolutely sure is to test but I would be very , very surprised if you had acquired HIV from the exposure you describe.  I have never seen a patient who acquired HIV in this way but I do know it happens quite rarely.  EWH---

Concern is not very high from what research I have done and I see that it is stated that oral has happened with ejaculation and gaping wounds in the mouth.  It was so brief (probably closer to 15 seconds), no ejaculation and I have a healthy mouth so sounds like it would be as close to zero as you can get.  I understand you cannot say “no risk” as there is always a risk with whatever you do.   But did see where Dr. hansfeld calculated risk like 1 in a million for a situation like mine.

My question that wasn’t answered is if I do an hiv rna test at 11 days (does it matter if it is 10.5 days) and it is negative, can I put the episode behind me.

I agree with your assessment.  thanks for doing the research.  If I were you I would not be the least bit worried.

As far as the HIV RNA tests, if you choose to do it at 11 days a negative result is proof that you were not infected and with that information I would move forward without concern.  I would not do further testing.

Ok.  I took the HIV RNA test through Labcorp at 11.5 days and awaiting results.  If negative, will put HIV behind me.   Probably more nervous than I should be based on research I've done.  Amazing how the internet and health professionals have such a viewpoint of giving a blowjob without ejaculation is from "no risk" to 1:2500 and everything in between.

Other question is in regards to other STIs.   I went to doctor 6 days after exposure as throat was scratchy.   They did a swab for Gonorrhea and Chlamydia which was negative; although they did treat me empirically with 1 shot and 2 pills as I was traveling the next day and would of had limited access to medicine if positive.

My strep test came back positive and doctor thought we caught just as it was starting.  They gave me a fairly large shot of penicillin (my choice) rather than take 10 days of penicillin pills.

I am assuming with the drugs they gave me, even if the swab test was inaccurate I was covered for Gonorrhea and Chlamydia.  I am also assuming the penicillin for Strep would also knock out any potential Syphilis.  Can you confirm?

Sounds like I just need to wait out my results for the HIV RNA test (should have by Monday at latest) and if negative - sounds like I am in the clear?
