I would like to know how accurate CMIA duo is 3 months after exposure and is there any reason to get tested using Nat PCR test. Thank you

All the HIV antigen-antibody (duo, combo, 4th generation) blood tests are conclusive about 6 weeks after the last possible exposure, even though some conservative information sources still say 3 months. In any case, there are no known false negative results at 3 months. Assuming you're talking about a negative result, it is conclusive and for sure you do not have HIV unless possibly if you were exposed more recently.

I have read online that some people never produce detectable antibodies, is that true and would it be smart to use viral load test?

First, that's an urban myth; if it actually happens, it is exceedingly rare. But even if antibody were not produced, the whole point of the AgAb (duo tests) is that they test for the virus itself (the antigen part). If antibody isn't produced, the antigen component of the test would be even more strongly positive than otherwise. That's why the AgAb tests are a significant improvement over previous HIV blood tests.---

So what your saying is one or the other would be reactive on the test. What if you had weak immune system would it still find the antigens of the virus. Is this test almost 100% at 3 months.

You're overthinking it. A "weak immune system" in theory could mean reduced antibody production, but that would make the virus MORE detectable. This test is 100% at 6 weeks and therefore also at 3 months.
