Hi Drs.


Is it true that people is not circumcised has more risk to get infections and stds?  (always use condoms)

If I’m not circumcised do I need to have any special care when I urinate, like retracting the foreskin?


Do I need to wipe or use toilet paper to clean myself after I urinate in order to prevent problems like balanitis (never had it, is it contagious?) , fungus or it is Okay just to just shake it and save it ? 


The normal care I have been having since I remember it is just retract the foreskin and clean with water and soap when taking a bath. 


I have these question since I had a conversation with my primary doctor who told me I need to clean after I urinate but my urologist told no need and that the moisture left after I urinate will no create any problem.  Sorry maybe these are more educational questions but I trust a lot in the experts opinion

Welcome to the foum. Thanks for your question.

Being uncircumcised roughly doubles the chance of catching HIV if sexually exposed to the virus. This is a big deal for public health and the frequency of HIV. In some countries, if all men were circumcised, the frequency of new HIV infections in men will be roughly half what it would be without circumcision. This is why circumcision is being promoted strongly in places like sub-Saharan Africa. On the other hand, "double" doesn't mean very much for most men in industrialized countries. Since you always use condoms, and probably few if any of your sex partners is likely to have HIV, your risk of HIV in the next decade might be, say, 1 chance in 10 million if you had been circumcised. Doubling that makes the risk 1 in 5 million. So despite doubled risk, your chance of getting HIV is zero for all pracical purposes. (Higher of course if you have sex with men or otherwise are at especially high risk of HIV.)

Your other questions all have to do with genital hygiene, not really an STD/HIV issue, so my comments are brief. As best I know, your own normal care (daily foreskin retraction with washing) is all that is necessary. How you urinate makes no known difference in risk of balanitis or any STD or other genital health condition. To my knowlege, the large majority of uncircumcised men do not wipe after urination and I see no need for it. In other words, I agree with your urologist.

Dr. Handsfield  thank you so much for taking the time to answer my questions and the information provided.
I initially asked my primary doctor because I noticed some redness in my glands but it turned to be nothing  since it got better by itself but He was the one that told me I needed to clean after urinate (which I have never done since I remember)and maybe think about circumcision.

My Urologist and Dermatologist  (same office) checked me and said it was normal redness and it will get better by itself which it did that  maybe I was just putting too much attention to this subject..  They told me I had no need to retract my foreskin and no need to wipe or clean that actually the toilet paper could create irritation.  

That my daily routine was enough.  I only have one partner (female) for years and never had any issues but just wanted to make sure and as I stated I trust a lot in your opinion.  
I wanted to have no doubts if I was in any risk by not cleaning and know I was not putting in any risk to my partner.

My only other comment is to agree with your doctors that a little redness of the head of the penis (mild nonspeific balanitis?) is a normal occurrence from time to in uncirumcised men. Even if you had overt inflammatory balanitis, it would not be harmful to your partner.
