[Question #5273] 7 day symptoms

Hi Docs,

Perhaps you could advise. 

Exactly 7 days ago: On the same night I had 4 episodes of protected vaginal sex with a 30 y/o woman from Brazil. We're still in contact now and she hasn't been tested in a few years. She said she will test this week for peace of mind, though she has a conservative past having had a monogomous partner for 12 years. 

She performed oral sex on me quite vigorously a few times and I fingered her, performing brief cunnilingus as well. The condoms did not break. There was brief genital on genital rubbing and perhaps some vaginal fluid got on the tip of my penis, I can't be sure but it was brief and she performed oral sex immediately after. I'm confident I did not penetrate as penetration on the whole was difficult when trying  actual.intercourdr - it was more genitals pressed against each other 

I've read around on the "risk" of various acts and I believe I am fine however the onset of some symptoms is concerning. 

Day 6: developed a sore throat with a mucus sensation when I swallow can be quite scratchy. I was congested in the morning but now nasal passages unblocked and still sore throat when swallowing. I rarely get sore throats

Day 7 (today):  Sore throat persists and I have 5 pimple like spots scattered  on my stomach/torso. 

1) Could these be interpreted as ARS sore throat/rash or more likely coincidental throat/ sweat spots from gym?  

2) If vaginal secretions came onto my penis tip (I'm uncircumcised) while genital.rubbing - is that  risk for HiV? Like I said she performed oral sex right after.

3)  Or am I correct in thinking if secetions  exposed to air/a non penetrating episode that wouldn't facilitate transmission. 

4) I very recently had a negative 4th gen ag/ab  test along with 2 negative rapids over the last 3 months. I hope I haven't undone that relief - I tried to be as careful as possible at the time. Would I be wasting my cash looking for a PCR/rna test?

I should add there was obviously deep French kissing within all this. Does this change anything?

Welcome to the forum.Thanks for your question.

It can be alarming to develop symptoms of any kind after a new sexual exposure or partnership. However, I am confident you do not have HIV from the exposure described. First, your description suggests your partner has no chance of having HIV. Second, the attempted vaginal sexual expsoures were condom protected, marketly reducing any risk of STD transmission. Third, with little if any penetration, there was little risk even without condoms. Fourth, oral sex is low risk for all STDs (much lower risk than unprotected vaginal or anal sex) and zero risk for some. For sure you were at no risk for HIV -- and it is exceedingly unlikely your partner has HV anyway.

In addition to all that, your symptoms are not suggestive of ARS. That you "rarely" get sore throats says nothing about the cause. Anyway, it started too soon for ARS; and other STDs rarely if ever cause sore throat. And neither HIV nor other STDs cause nasal congestion. You caught a cold, that's all. You might have caught it from your partner -- if close enough for sex, obviously you could catch a cold virus if she was carrying one. I can't say what to make of the skin "spots" you have noticed, except this also is not typical for ARS.

To your specific questions:

1) This does not sound at all like ARS. The symptoms are wrong and they started too soon.

2) Without actual vaginal penetration, HIV would not be transmitted. Even with unprotected vaginal sex to completion -- several minutes with penis inside vagina -- the average HIV transmission chance is once for every 2,500 exposures. On top of that, you used condoms, with a partner who almost certainly doesn't have HIV. No risk at all.

3) Air exposure of genital fluids or HIV makes no difference one way or the other. Urban myth.

4) Hmm...  I wish you had said at the start that you have already been tested for HIV. (Or that I had read all the way through your questions before answering the ones above.) No matter how high the risk for HIV was at the time of exposure, a negative AgAb test more than 6 weeks after exposure is proof you do not have HIV. Test results ALWAYS overrule symptoms, even typical symptoms -- which you did not have.

As for french kissing, it is risk free for HIV. But as just stated, the risk at the time of exposure doesn't matter.

So all is well. You don't have HIV and do not need further testing. I hope your cold has cleared up, however!

Thanks for your kind reply, good morning reading.

I should say that my tests were a week before this event - but I don't think this changes any of the rationale?  

Even if somehow she were HiVpositive; the nature of the acts and condom use  mean not to worry?

1 more facet- there was some condom based penetration, now that it was brief for a few minutes each time. Genital rubbing was without condoms

Sorry if I misunderstood the timing of your tests (and still not sure I understand):  you wrote "I very recently had a negative 4th gen ag/ab  test along with 2 negative rapids over the last 3 months." I assumed the 3 month time frame meant after the exposure you are concerned about. Obviously your tests before the event mean nothing in regard to that exposure.

And yes, you correctly interpreted my comments. No risk and no worry even if she had HIV -- which almost certainly she did not. But if you remain concerned, why not discuss it with her? You might find she is just as worried about HIV from this event as you are. Why shouldn't she just as worried about your HIV status as your are about hers? Even with your negative tests, maybe she doesn't trust that infrormation. If you both get tested now, you both would be 100% reassured that neither was a risk for the other. (I'm not necessarily recommending this -- but it could be an option if you remain concerned. But it seems it would be unfair to ask her to be tested and not do it again yourself.)

Thanks Doc; access to your advice is much appreciated. We agreed to go in a few weeks. In regard to persistent sore throat, quite sore on muscles around neck as well and mucus on swallow - I guess the absence of fever, aches, body rash is reassuring in itself? 

Given that it started on day 6 instead of day 7, is 24 hour earlier onset of symptoms long enough as it's only the space of a day? 

That'll be my last question(s) - I promise!

Correct:  sore throat alone, without other symptoms, is unlikely to be from HIV; and yes, it started too early.

You may also be reassured to know that in the 14 years since I (and later Dr. Hook) have been doing this and a previous online forum, with thousands of questions like yours, there has not yet been a single person who asked about HIV after a worrisome sexual exposure turned out to have caught HIV. You don't be the first. If and when it happens, I'm confident it will be a user with a truly high risk exposure (like unprotected sex with a known infected person, the regular partner of an infected person, or a guy who hand unprotected anal from another man).

So do your best to not worry at all. Have an AgAb test at the 6 week mark, but otherwise go about your life as normal, including sex with a regular partner if you have one. There's no realistic chance you have HIV.
