[Question #5358] Follow-up

Hi doctor. 

I really didnt want to come back again and keep thinking about this, but i don't want to keep analyzing myself. 

I started unprotected sex with my wife after exposure almost in first of Nov 2018. Since then, she got sick around 3 times (Dec, Feb, and now May) all with fever and sore throat. 

The last time lasted almost from 5 - 10 may (6 days) where she said it's one of the worst flues ever which scared me a lot 

On 5 may, her temp was around 37.5 C until 7 pm then became high to 38.4 C and maybe around 39 C (with muscle aches for few hours around legs area) until she sweat a lot at 4 am and all was fine. But was really tired. 

The next 2 days the temp was 37. 3 C or around max and the 4th day back again to 38 C from 10-2 oclock then fine for the rest of the days.

There was sore throat for the whole period and difficult swallowing (mid to severe). The doctor said she had "tonsillitis". She also was snoring and barely breathing from nose during that time (not sure if it is nasal polyps) as she felt bad around the nose area. 

Please note doctor she didnt take antibiotics until the temp went high again (38) as we suspected she was pregnant.. And she was fasting as well (which may affected her sore throat). No rash or diarrhea or lymph nodes. 

Sorry for the detaila doctor but im really worried and cant keep to to myself.

Sorry forgot the question.. Can this be ARS.. I hate to ask this but i just need to be on the clear and have everything obvious.

Sorry i keep remembering stuff.. She also has pressure in here ears. Does it prove anything?

I'm sorry that you continue to worry about the no risk events that you took part in over 6 months ago.  Your concerns are not well founded.  You were not at risk for HIV from the exposure you described previously.  Your wife had an upper respiratory tract infection of the sort that most people do have from time to time.  Pressure in the ears is not a sign of HIV or the ARS

On our last interaction I told you that there was  no reason for you to test.  At this time, while there is NO medical or scientific reason for you to test. I wonder if it would be useful for you to test at this time to prove that you did not acquire HIV from your exposure.  When your test is negative, as I am confident it will be, then you will have proven that you could not have given your wife HIV.  

I hope these comments are helpful.  Clearly you still feel some guilt about your encounter.  If you cannot stop worrying, I would suggest you speak with a mental health professional to get assistance in dealing with your guilt.  EWH

Thank you doctor for your reply. 

I try to always be calm about ut it but when she geta sick and has fever and sore throat i strat to panic.

I admire your experience and trust what you say for sure, but can i know why you suggested it is an upper respiratory tract infection although she had high fever/ severe sore throat and joint or muscle aches.. 

I actually tested in 3 different labs from Nov 2018 to Feb 2019 using 4th gen (CLIA - ECLIA - ELFA)0and all negative in good famous labs in Dubai. But i saw while googling someone saying that some labs give negative result to everyone so i keep panicing from the symptoms. 

Another quration Dr. Hook, when someone is inflected, does the immune system gets weak fast.. Or it stays normal after the ARS until the advanced stage (AIDS). it's the first time i see my wife getting sick that much.. 3 times in 5 months

There was slight coughing.. Sort of nose congestion without discharge and no runny nose.

Flu-like symptoms often accompany upper respiratory tract infections.   The tonsils are part of the upper respiratory tract.  In addition pressure in the ears is likewise a symptom of upper respiratory tract infections.

i was not aware that you had tested in the past.  Your negative tests likewise PROVE that you do not have HIV.

As I told you before, googling these things will only fuel your guilt-driven anxiety.

When persons acquire HIV their immune system does not weaken profoundly until years after infection.  On average, it takes 9-10 years to progress to AIDS.

Now Doctor i know it's my last reply, i just would likebto address some other concerns (and thanks for ur reassurance).

1. Isn't the high fever (around 38.5) alarming? She was so hot when i touched her. 

2. Is it common in ARS for fever to go up and down? She had fever 38.5 fot hours then no fever for 2 days then 38 for 2 hours. 

3. Do the pain around her nose, pressure and sounds in ears like waves, and tonsillitis mean anything against ARS? 

4. She almost had URTI before two months wigh nose discharge and cough (she has little cough this time too). Is it possible yo have URTI again in short time? 

5. Are all the 4th gen tests done in lab (ECLIA, CLIA, ELFA) the same abd Fda-approved? As they are invented by german and french companies not like CMIA (i didnt find in Dubai) which is American. 

Sorry for my tension but the fact that ARS in different from one person to another makes me in tension also.

1.  A fever of 38.5 can have many causes.  The ARS is statistically among the least likely in your situation so, no, I am not concerned about this temperature.

4. Certainly it is possible to have two URIs over a short period of time
5.  While you do not mention the manufacturer, the labs in Dubai are very high quality and I suspect the tests you have had were FDA and WHO approved.
