[Question #5557] Herpes risk assesment

Hello Doctor,
Please asses my risk
1) Protected sex with 3 CSW in 2014 and i have given oral to one of them. No symptoms or i might have not noticed.
2) 2017 December went to massage parlor and ended with handjob with massage oil. Any risk here because one week painless on penis foreskin. after applying the steriod cream it was gone but after one week penis foreskin too much itchy and white head pimples came on glans when i touched white head gone and red rash was there. I went to the dermatologist and he ruled out candida balanities and gave canasten & fusidic cream with these there was some relief but these symptoms were there almost until September 2017 and cleared up everything. Have done Elisa igG testing was done on April 2017 with negative result.
3) August 2018 went to massage where we both naked and my penis touched her pubic area for 10 sec but penis rubbed her pubic are with towel. There was no direct rubbing between us. Is there any herpes or STD risk in this case. Because had this kind of encounters with 4 others. There was towel between while rubbing the genitals. After this incident had rashes over glans almost two months. i visited the dermatologist initially he said it was fungal but my next visit he said rashes looking differently and he gave me acivir 400 tablet for 5 days and rashes gone but they appeared again and i went to same dermatologist and this time he gave me different tables and when i asked was it herpes he said no. I am totally confused.
4) Any risk if she sat on my backside and rubbed on my back, buttocks and legs. is there any change herpes transfer this way?
I have done Elisa igg testing and all came negative except one which was done on May 5 and the index values are 2.51 for hsv 1 and 0.96 for hsv2.
After that i have rested at same lab after one week and result was negative with 0.65 for hsv1 and 0.54 for hsv2.
My latest test was done on June 10 and result was negative which covers 4 years for first incident, 1.5 years for second inc

So have you ever had a cold sore on your lip or in your nose? The one test on May5th is a decently strongly HSV 1 positive result - in general, the IgG tests misses 30% of HSV 1 compared to the gold standard western blot so that could be why the subsequent ones were negative. If you want greater clarity about your HSV 1 status, you could obtain the herpes western blot.
In terms of risk from the encounters, I think you were extremely careful and I see no real risk in the behaviors described above. So if you are truly positive for HSV 1, I doubt that it is from these encounters. Let me know

Thanks for reply.
I never had any cold sore in my lip or nose. What about hsv 2 from above incidents. Can I move on with these test that I'm negative for hsv 2 ? My last exposure was 7 weeks ago , there was genital rubbing with towel between us.
Recently got married and had intercourse with my wife using protection. After 15 days she got cold, fever, body aches but there were no cold sores on lips/genital area. Could these symptoms related to herpes? Can I continue intercourse without protection with my wife?
Or should I wait for 12 weeks and have one more test to continue unprotected sex.
My main worry is hsv2. Please let me know the risk factors for hsv2.

I don't believe that her symptoms were related to herpes, no. I think you can safely have intercourse with your wife, yes.
There is no risk of HSV 2 here because there was a towel between your genitals and her genitals. You need direct contact to get herpes and this just didn't happen with you.

Hi Terri,
Can you please clarify below questions.
1) I have latest test results with two different labs today.
Elisa igG hsv 1 0.30 and Hsv 2 0.40
Another lab Hsv 1 >1.0 U/mL hsv 2 also > 1.0 U/mL
It was also negative.
Can I say I am hsv 2 free with above tests or do I need to further testing?
2) Let's say I got 2.51 hsv 1 and 0.96 hsv 2 on May 5 and assume that I have hsv 1 and my could have got by this time?
3) my only direct contact penis to her pubic not more than 10 sec. It was only touch and rubbing happened with towel. This happened with two different ladies. That 10 sec touch has any risk?
4) what do you think symptoms had earlier? Was that fungal infection? The doctor gave me acivir 400 mg tablet on October month that is concerned me. Herpes rashes stays two months continuous?
Do I need to go for Western blot testing?
I am from India. How can I obtain western blot test? I am really not sure how to send blood sample to there. Can you please help with this. Is there any way we can order test kit and conduct test any lab here?

I don't believe that the 10 sec contact presented a risk, no. Herpes rashes do not stay two months, no. I don't think you need western blot testing UNLESS you want to clarify the HSV 1 issues which is discrepant between the two labs.
