[Question #5615] Unprotected Oral

5 days ago I received unprotected oral sex from a woman , I am a man.  I’m worried about possible diseases , including herpes , which I had been negative in past.  We kissed as well and had protected vaginal sex.  Very anxious and worried now.  I know  some say it’s low risk, but obviously it happens to people.  As of right now, no sores or blisters.

It does happen to some people, yes.  Have you ever in your life had a cold sore on your lip or in your nose?  If yes, then you already have HSV 1 (I know you said you tested negative in the past but since the HSV 1 IgG test misses 30% of infections compared to the gold standard western blot, I always ask about symptoms).  The average time from infection to symptoms is 5.5 days with the range being 2-10 days.  So you're half way out of the woods IF you are going to develop symptoms with infection.

I’ve had canker sores , but nothing on lips or outside.  I am an anxious person to begin with with and I’m extremely frustrated with myself.  So far , no symptoms besides high stress.  I’m planning on getting full round of test again but would like to ask if at 4 weeks would that cover HIV, herpes, etc?  So if no sores after 14 days , is that a fairly good sign of no infection ,and even though very low risk,  4th generation test at 4 weeks would be ok?  Thank you

What are odds ( probability) of oral transmission even if woman did indeed have HSV1? And for HIV?

If no sores at 14 days, there is an excellent chance that you are not infected with herpes.
I think a 4th generation HIV test at 6 weeks should be a final result for herpes.  We like longer for a final herpes test - 12 weeks is best.  But the IgG test for HSV 1 misses 30% of HSV 1 infections, so not a great test, compared to the gold standard herpes western blot.  And HSV 1 would be the concern here.
We don't have statistics about oral transmission of HSV 1 but I know that Dr. Handsfield would tell you that oral sex is not a risk for HIV transmission.

Okay - last question.  If were to experience tingling on my lip , is it just the spot where a potential sore would be , or my whole lip?  Feel like both of my lips are starting to tingle ,but like I’ve stated, anxiety is very high right now.  Thanks for your service.

Usually, when people have oral herpes, tingling happens in a specific spot, not the entire lip.
