[Question #5649] Genital itching and redness after sexual exposure

Hello Terri, I have had sexual encounter with a female I am assuming she is herpes infected (csw). Following the encounter (protected oral and virginal) I started itching terribly in scortal, pubic, anal and perineum areas. Tested negative for both hsv 1 and 2 at 4,7 10.5, 18.5 weeks and negative western blot (from UoW) at 6.5 months. Negative at 15 months again for igGs. 

I am terribly itching once in a while and following application of steroid cream areas gets very irritated and reddish when I stop the cream the redness reduces and I have seen few physicians during such situations and no sores were found but very reddish areas in my Perineum and anus. My wife is asking now to have sex but I am extremely worried to have sex as I am concerned that something is being missed here and she wants to get conceived. 

1. Do I have to be worried to have sex?

2. Itching returns once in few months in anal areas(worried it is an outbreak but no sores). Can an out break show up like this? 

3. Can I wear an underwear protecting my genitals and have unprotected sex with my penis out alone? I have no like zero itching on my penis.

4. Should I start on antivirals and see if this gets better? I am struggling to say anything to her (this is killing me inside). Pls help. 

5. Should I get PCr swab if redness and itching returns in anal area? 

6. Why would an unprotected wed result in itching in perineum and anal I am stumped. I don’t think the ssd lasted even five min. Can u clarify?

1.  No.  You should not be worried to have sex.  The western blot is the best antibody test for herpes and its negative.
2.  No.  This is not the presentation of herpes
3.  I have no idea why you would want to do that.  You don't have herpes
4.  No.  There is no need to start antivirals.  You don't have herpes
5.  No.  I see no reason to take antivirals
6.  The person in this post did not have a negative western blot, at least as far as I can tell.

I'm wondering why you seem so set on believing that you have herpes.  I suspect it's because you have guilt about your experience and are finding it difficult to believe that nothing negative came out of it.  What do you think?
And I am assuming that you've tried treating the itching with antifungals?

Sorry for not being clear on my post Terri. 

That was MY post and the way I have written in #7 is not clear I reckon as I read it again. That was how my anal area and perineum was. Any comments? 

I treated with antifumgals but I don’t think I found any difference.

The itching reduced quite a bit compared to last year but the itching shows up in my pubic areas very sporadically and early morning as soon as I wake up I get itchy in my balls.

The reason why I am so much believing that this is herpes is that I saw few posts on your Westoverheights and the above forum where few people had negative western blot some but they swabbed positive which makes me go crazy however they seemed to have sores but I did not. I just get redness. 

What would u advise on the next steps if I get itching and redness in genitals. Genital itching seems to take away most of my time and I keep worried. Just worried of infecting my wife.

I know of only 10 people in 36 years now who were positive by HSV 2 swab test and negative by western blot.  This is not you.  You are not swab test positive so you should not compare yourself to them.  I don't know how much antifungal you have tried or in what form but this sounds exactly like what a fungal infection would do - periodic itching, often in the AM after a night of covers, making the genital area warm and moist.  Your symptoms do not sound like herpes and you need to believe the blot.

Hello Terri,

I used lortimine cream not for too long (less than 10 days) followed by fungal medicines however not a lot of them. 

I still suffer with itching during exams and it grows more when I feel more stressed both the times It happened when stressed in anal and groin area only itching seen and urge to itch there and associated redness and pain and irritation 

I have red few posts who tested positive via swab and negative on Western blot thru Westover heights and claim that they spoke to you but I don’t see updates if the person

-pls read post from Birk.neg25 scroll almost 3/4th of the page who tested negative for WB and positive for Hsv2 which u later questioned but don’t know if it was proved positive again or not..?

There are so many factors to consider when reading these posts online.
The western blot is 96% sensitive for HSV 1, so less sensitive than HSV 2.  Important to keep in mind
We can't tell for certain if the individuals involved had had sex during those waiting times with anyone else - some people don't consider receiving oral sex "sex".
We don't know the impact of antiviral medicine on these people - how much they have taken and what dose and how close the dosing was to a new infection.
I always try to tell people - if you test negative by the blot but have obvious blisters in the genital area , get them swab tested! 
The thing is, with as many people as I talk to who have negative western blots but unwelcome irritating symptoms that they are convinced are herpes, it just can't be the test is missing all of them!
