[Question #5737] Confused about HSV test results

I was diagnosed in Jan of this year with HSV 1+2. I have had only three partners total, and most sex has been oral. I was experiencing discomfort in my genitals so went to the doctor and what I hoped was razor burn or general irritation was swabbed and then I had blood work.The results came back positive for HSV1 and 2.Horrified, I began to read everything I could find from reputable sources (CDC, etc) and learned that the testing I had from Quest may have been unreliable. 7 months later I requested new tests for comparison.The numbers don’t make sense to me and I really want to know what I have. Jan 2019: rapid culture ISOLATED/HSV1 IGG 26.8/HSV2 IGM 1.32, IGG <.90. July 2019:HSV1 IGM 1.20, IGG 10.0/HSV2 IGM 1.20, IGG <.90) I don’ see evidence of HSV2 in either IGG test,only the IGM,and that is said to be unreliable. Can you please clarify. My doctor seems to be focused on the IGM results.

So the culture grew out HSV 1, if I am understanding your results correctly and you had a high HSV 1 antibody level which means that this is not new infection for you. Ignore the IgM results completely. The CDC says never to use it. So from what I read here, you have HSV 1 genitally, which is likely to recur infrequently. In my experience, the IgM is falsely positive about 80% of the time (i.e. the IgG never becomes positive). It was an error to have drawn it (says the CDC)

Thank you SO MUCH for your reply. I have several more questions please. 1) Is it the word ISOLATED that mean the virus was found in the swab? 2) You said high IGG numbers mean it isn’t a new infection. What kind of timespan do you mean by NEW? Months, years, since childhood? 3) What does it mean that the IGG went from 28 to 10 in seven months? 4) Should I accept the HSV1 positive result and HSV2 negative result based on the IGG, or is there any reason to retest at some point? 5) What sexual precautions should I take going forward with genital HSV1? I can’t thank you enough for sharing your expertise. Such a scary time for me.

Isolated does mean virus was found in the swab, yes.
It could have been months, years or since childhood, yes, any of these, no way to know
It means nothing - it is normal for it to vary or if different labs were used for the draw, it is normal to vary as well.
I think you should accept this yes.
You may wish to consider taking daily antiviral medicine if having sex with someone who is actually HSV 1 negative.
