[Question #5835] HIV risk

Hello, first I am a 40 year old circumcised male. I had 2 encounters with Asian massage parlor gals I would like your option on.
1st event while in Las Vegas July 6th I received unprotected oral sex and protected vaginal sex that lasted maybe 4 minutes. Followed by a couple minutes of protected anal sex when I came to my senses and thought this may be more risky then I wanted(maybe 5 or 6 (thrusts). So she removed the condom and finished with unprotected oral. There was a few deep kisses. After we were finished I showered and left
2nd event while in Chicago July 15th an Asian masseuse sat on top of me for about 3 minutes for protected vaginal sex. I showered and left. The next day I started to panic and went to my family doctor and explained and received a shot of something and 1g of Zithromax. He said that would address most of my issues until I could be tested at later date for hiv.
I have had no ARS symptoms or breakouts at all. On day 32 from event 1 and 23 days from event 2 I had a hiv comb gen 4 test done and a RNA hiv blood test the same day. I will post results when I get them.
Q1) if these tests come back negative would this be conclusive for HIV
Q2) would the shot and meds given have addressed any possibility of Syphlis. Would you test if you were me at 6 weeks?
Q3 would you have even recommended testin for hiv under these events.
Q4Should I have just resumed unprotected sex with my girlfriend and not been tested at all for hiv

My first reaction is to congratulate you on having safe sex! These were definitely low risk exposures for all STDs. The highest risk event was the unprotected oral sex, but oral sex is low risk for all STDs and virtually zero risk for some, including HIV. You (and your doctor) overreactied by treating you with ceftriaxone (the shot) and azithromycin. They indeed would have prevented gonorrhea, syphilis, chlamydia, and nongonococcal urethritis (NGU). But you were at little risk for any of them. And the vast majority of commercial sex workers do not have HIV (regardless of ethnicity -- being Asian doesn't elevate the chance your partners were infected.)
1. Your negative test results, when you get them, will be 100% conclusive for the exposure a month earlier and nearly so for the one 23 days before the tests.
2. See above. There is no point in syphilis testing at this point. Even if you had been infected, treatment so soon would have cured it and prevented any positive test result. Don't waste any more money on testing you don't need!
3.4. From the standpoint of risk from these events, I definitely would not have recommended HIV testing. But I often recommend testing for reassurance even when risk is near zero. Many persons worried about particular exposrues are more reassured by negative test results than by expert opinion. So for reassurance, it was fine for you to be tested. But from a medical/risk standpoint, it wasn't necessary. If somehow I had been in your situation, I would not have been tested and would have continued unprotected sex with my wife without worry.
I hope these comments are helpful. Let me know if anything isn't clear.

Thank you Dr. Hunter
My test results came back this morning and the Gen 4 test was non reactive
The HIV 1 RNA Qualitative test was negative as well
I will take these 2 tests as a conclusive enough result and more on.
