[Question #6044] Been worried for a while.

Respected doctors,
I will try to keep this as short as possible.
Back on  August 11 of 2017 I had protected sex and oral  her to me from what I saw but it was dark and i didn’t inspect the condom with a prostitute (wasn’t high class) some strip club around my area in CT.  It was about 7-9 min and I’ve been scared i contracted hiv ever since she provided the protection .  I’m married, and before I resumed sex with my wife i took an oraquick home swab first week of January 2018 it was negative.  We had our second child after she was tested during pregnancy negative but I’ve been freaking ever since every time she gets a cold or when the baby got cradle cap or when he gets a fever that I passed it to them. I honestly am scared as I type now. 
I just did a full std panel recently and it was a doctor/ rn i told her my story and she was like “you could have gotten many stds from that including hiv”
My question are as follows.
1. Can I trust that one test? it was prob around 4.5 after.
Do you guys think my 4th generation test which I’m waiting for results for its been days since they send it to quest...will be the same as the oraquick?
3. What were my statistical chances of catching hiv that night?

I know you two are some of the most educated people on this topic.

A little bit more info, I don’t remember getting sick at all. But recently my dandruff has gotten much worse and she was diagnosed with “dry scalp” though she is breastfeeding and her and I only had sex two times after the incident and she was tested before the second  time we had sex.  The woman was fairly young and the condom didn’t break cause I grabbed the tip to take it off and it was intact but I did not ask her about her hiv status because of language barrier.  Also I’m 34 years old.. I freaked out so much I lost weight but then put more weight back on after the negative oraquick test.
She’s been sick a lot, I’ve been sick though we do have kids one in pre school.  None of this is prob relevant but I figured I’d add it in. I also
Know the baby was checked for “antibodies” for hiv at birth with the bloodspot check.

Sorry I didn’t realize questions were deleted and not answered by making a second window. I apologize and understand that rules are rules.   Obviously I would appreciate the reply when either of you have time but if you don’t I understand.

Welcome to the Forum. Thanks for your question.  I'll be glad to help.  Let me start by making several general comments:
1.  Most people, even most CSWs, do not have HIV.  HIV is rare in women, even if they are CSWs, occurring in less than a fraction of 1% of women.
2.  Condoms remain a highly reliable means for prevention of HIV.  If your condom had failed, you would have most likely known it.
3.  There are no known instances in which a person has been proven to have acquired HIV from receipt of unprotected sex.

Thus, your exposure was quite low risk.  Even if your partner had HIV and even if the condom had failed (I have discounted ANY risk related to the oral sex), your risk for acquisition of HIV from a single unprotected exposure is substantially less than 1 infection for every 100 exposures.  

Thus, in answer to your questions:
1.  OraQuicks are our least favorite tests for HIV. They are slightly less sensitive than other tests, they are highly reliable and between the low risk nature of your exposure and your negative result, you can be confident that you were not infected.  
2.  I am confident that your 4th generation test will confirm that you did not get HIV.
3.  See above, statistically your risk of having been infected is in the neighborhood of 1 in a million or less.

I just want to say you and your partners are worth ever penny and some thanks for the response. Just have a follow up question.

So you agree the Oraquick  results were  accurate in my case at  4.5 months even if it is your least favorite?

Do you think I could have picked up any other stds or STIs and not had any symptoms or noticed for 2 years at this point?  I’m not realky talking about Hpv or herpies but other stds with no symptoms.

Also this woman wasn’t high class either doesn’t change your assessment?

Yes, I would believe that your OraQuick test was correct.  It is for that reason that I stated that I anticipated that your 4th generation test will also be negative.  

Regard to other STIs, it is very unlikely that you acquired anything and that it remained asymptomatic all this time.  Your risk for herpes was non-existent and the question of HPV is irrelevant since over 80% of U.S. adults already have it, whether or not they know it and because acquisition of HPV following receipt of oral sex is quite uncommon.  

That your partner was not "high class" in no way changes my assessment.  EWH

You also meant nobody has ever been proven to get hiv from protected sex not unprotected  I think it Was a typo.

How long do you think the full std panel including the hiv and urine test will take to receive results?  I went last Monday and it’s now Sunday.. woman told me Friday but I never got a call, they did have to send it to a lab which was called quest.  
Could it take a week sometimes? 
How long does a 4 generation test take in general to get results by itself?

I left the thread open because I was hoping you would get your result and post them.  This will be my final reply.  Our Forum provides up to three responses to each question and therefore, as per Forum guidelines, this thread will be closed shortly.  In response to your questions:

1.  My statement, "There are no known instances in which a person has been proven to have acquired HIV from receipt of unprotected sex." was not a typo.  There are NO proven cases in which someone has acquired HIV from RECEIPT of ORAL SEX!

2.  The performance of the testing for STIs or for HIV once the specimen is in the laboratory takes only a few hours.  More time is required for specimen transport, reporting, etc.  The Quest Laboratories are a well known, highly respected commercial laboratory.  Obviously I cannot tell you why getting your result has been held up but that does not reduce my confidence that the activities you have described would not lead to you having an STI.  I suspect the hold up may have more to do with the doctor's office than the laboratory but of course that is just a guess.
