Hi Drs amazing site thanks for helping us all!
My situation I’m sure is the same as everyone else’s. I made a mistake 140 days ago and tested out to 135 days hiv 4th gen duo blood from a vein .... also at 4 weeks (clinic and lab) and 6 weeks (postal sample 5th gen).
I had oral and protected vaginal , no idea if condom was a good make etc.....I have experienced symptoms, sore joints, lymph nodes in back of neck come and go and sore collarbone but have been prodding them and my jaw line....I can definitely feel something they aren’t huge...(back of neck more prominent). Consistent runny nose and dry cough (mostly mornings)...also sore left rib cage (spleen not enlarged had ultrasound) I have tested too many times always neg....immune system potentially a worry with cvid in the family....low (ish) RBC 4.38 and other bloods pretty normal ...rheumatoid factor 20 ..wbc 4.8 lymphocyte 1.5, platelet 210
Anxiety I know is an issue ....if I have a suppressed immune system could I be false negative?
Do I go for a pcr or naat or just man up and be safe moving forward!? Thanks so much

Also I had spine surgery at around 2 and a half months after the incident....could that delay production of antibodies of lend weight to a false negative .... bearing in mind I had tests before this maybe not?!

Welcome to the Forum. Thanks for your confidence in our service. It is time for you to accept that you did not acquire HIV from the virtually no risk exposure that you describe (most people do not have HIV, most unprotected exposures do not result in infection, no one has ever been proven to acquire HIV from receipt of oral sex, and iwhen condoms fail, they break wide open. Instead, I would look for other reasons for your symptoms. If you had acquired HIV, even if you had a faulty immune system and we're not producing antibodies, the antigen detection portion of the fourth generation test would be strongly positive and diagnostic. There is absolutely no reason for further testing related to the exposure you are concerned about. EWH---

So for my untrained obtuseness.... P24 test on the duo is sensitive enough to rely on ... and the absence of an aggressive antibody reaction would mean p24 antigen would remain detectable as the abs haven’t destroyed the p24...in short my tests are conclusive .... FYI gamma gt is raised in isolation and I’m seeing a specialist to identify the cause...can’t help but think it is all linked...

Your summary is correct. If your immune system were not producing antibodies (something that I find VERY unlikely if you have reached adulthood without multiple life-threatening infections) viral concentrations in your blood would be very high and readily detected by the antigen detection component of the tests.
Please note, I am not disputing your symptoms. At the same time, I am confident that they are not due to HIV acquired from the exposure you describe. Rather than needlessly expend energy pursuing something that is NOT there, I recommend working with your doctor to find out what is going on. EWH

Yup happy to close, FYI lymph ultrasound no real alarms, all blood tests report no immune or autoimmune disease... will control anxiety moving forward .. thanks again.
