[Question #6265] Herpes/ scare(pretty sure i’m positive for hsv)

So long story short first possible exposure 38 days ago I had sex with a lady I picked up on the streets, it’s in a horrible area where many crack heads and Injecting drug users live, It was protected sex, but unprotect oral, second two days ago, two random ladies from the same area, one had meth sores,
Only unprotected oral no sex at all. Fast forward to 7-10 days Later, I have burning pain when I urinate, is started on Friday, it is not Monday, and no visible sores, it has gotten worst every day, but still no visible sores. I have had many herpes/ hiv scares before, and you 3  have been goods to me, so trust me I have nit picked for symptoms before)And there is no denying this very painful pee burn. 
1) My encounter sounds spot on to when Dr .hhh states Herpes symptoms show up, what’s the chances I aquired hsv1 or 2
2) if I did Indeed encounter Hsv, what are the chances I also aquired hiv in the process?
3) I also forgot to mention , I fingered the second lady, that had some meth looking sores, and I finished my self  off with masturbation, could that have been how I got Hsv( I believe it was with the opposite hands, but not 100% sure, )also I was drunk, and it was in my car at night, so I could not see if Any visible sores or not on any enounter 
4) If the herpes is just inside the urethra, is there a culture my doctor can do to see if I have been infected(because no lesions outside yet.
5) Am I still in predome? Maybe the sores haven’t appeared yet, even tho it has been 3 days already, how long after symptoms do sores usually appear? 
6) I am going to a clinic tomorrow, haven’t been able to since I was out of town, should I start taking antivirals and antibiotics to stop the burning urine? How do I stop this burning pee, I can’t take it anymore, im drinking alcohol, to alleviate the pain alot, it doesn’t hurt much when I drink alcohol, please help!
Even tho I am going to get stds check, which I will post here later, I have gone to former doctors/dermatologists for std sc

While it is possible that you acquired HSV 1 from the sexual encounters that you had, it seems very unlikely to me given that you've had no external sores at all.

Since your intercourse encounters were condom protected, I do not worry about HIV acquisition in your situation.

No, I don't believe that this is a risk for you acquiring HSV and certainly not HIV

Yes, a swab test can be done of the inside of your urethra.

5) Am I still in predome? Maybe the sores haven’t appeared yet, even tho it has been 3 days already, how long after symptoms do sores usually appear?

I would say that it is likely that if you have HSV 1 genitally, sores would have appeared by now.

You should have a full STD screen and do not start taking any medicines until you are seen, this can confuse the test results.

First Off I would like to thank you for the reply, and I am here for my follow up question. I just got out of the Doctors, and told him every detail, he does not seem to think it is herpes , and really paid no mind to it the second He saw that there were
No lesions, he prescribed me antibiotics to take for a uti, and I got a full std panel, he said he preffered I take these first was prescribed “ Bactrim” and if One of the stds come out positive, to start taking those pills for chlaymidia or Gonorrhe on my follow up appoitment if the out positive,  unfortuunately I will not receive  the results until Monday, which is 6 days from now, seems like an eternity.
1) I’m still worried I may have contracted Hsv, does there really always have to be obvious lesions for it to be herpes, are there many cases where one has it, but no obvious Lesions?
2) He seemed more convinced it was A uti or Gonorrhea, do you agree with his stance, or do you still believe that hsv whethere 1 or 2 is a possibility from any of the encounters? Just the timing seems so weird for me to coincidently get a uti at this time, at the perfect timing of when hsv symptoms would show up from my two encounters on the same day 11 days ago.
3) I currently have a sexual partner, and she got tested two months ago, and everything came out negative for her, and she has never had any weird symptoms she claims, i’m scared to possibly pass something on to her, and I told her of my symptoms, and she still wanted to have sex, and we did yesterday, but I persuaded her only if I use a condom, no oral involved, could I have possibly passed anything to her now? 
4) If it is a uti can I pass that to her during sex, I believe my Doctor said no, but can’t remember
5) Should I be avoiding unprotected sex with my girlfriend from now on?
6) What is the difference between genital hsv1 and genital hsv2 ?is one worst that the other? Does one cause less outbreaks?

Hey sorry to bother, i’m sure you have been very busy Terri, and just wanted to post this, to see if maybe this question was missed, since it was posted 2 days ago already, I hope this doesn’t count as my second follow up question, as I would like to have my last one for when I receive the results on Monday, thanks again, and I apologize for the bother Terri, just this anxiety is killing me.

Dr Hhh or Hook You guys are active, do you think maybe you guys can chime in here, I’m sorry to sound mean or annoying, I understand Terri may be busy, and don’t want you guys to hate me, just it has been over three days and still no reply, And you  three are the only ones I trust the most with these questions since I know you three are some of the best when it comes to this

You can contract herpes without lesions, yes, but for someone looking very carefully as you are, that is unlikely.

I do actually agree with your doc on this.

Condoms reduce transmission of most everything, and for herpes, it reduces transmission by 96%.  So good for you for using condoms.

It depends upon what is causing a UTI.  Actually, urinary tract infections in men are rare. 

5) Should I be avoiding unprotected sex with my girlfriend from now on?
I think if you've been tested for everything at the appropriate timing intervals and are negative then it seems reasonable for you to resume sexual contact with your partner.

6) What is the difference between genital hsv1 and genital hsv2 ?is one worst that the other? Does one cause less outbreaks?
HSV 2 recurs far more frequently than HSV 1 genital infection and is shed more often and is more likely to be transmitted via intercourse

Hey thanks for the reply Terri, I was starting to feel better , but now I am completely  freaking out that I may have aquired Hiv please help!!

1)Starting on Thursday I started getting some weird symptoms, the first being I started to get about 8-10 red random dots on my upper chest, It was also there on Friday and then left, I have had many hiv scares before, which is why I always use condomds, so trust me I have looked before in those situationa and never have I seen these red dots, could that be an ars rash? The only other thing that I could think of is the”bactrim” that I am taking but thats the only thing saving me(Thursday was 13 days/ 2 weeks, after the 2 unprotected oral encounters.  Also forgot the lady joked and said that she had hiv after we were done, but said she was kidding, but I don’t believe her, because she lives in a high risk area, and those injecting drugs.

2) Friday I started to develop swollen glands under my neck, and the ones on the side were pretty tender, I have checked for them many times before, and these seem as tender as I have ever felt them, another ars symptom you believe? 

3) On Friday( 14 days after) I started to get really stuffy, turned to Saturday (15 days after)the stuffiest I can ever recall being in my whole life, I lieterally could barely breath, even my chest was hurting , i’m assuming from congestion, had a horribke headache as well, and a month ago I got sick, and it has never even been remotely this close.

4)I started having a slight sore throat which has gotten worst, and now it’s Sunday , And I just went to look in the mirror, and I have a bunch of white stuff, im assuming it’s oral thrush, I never ever get sore thoats, in in other hiv scares, never got it, which is usually what used to keep me sane, I heard that the white stuff on your tonsils is usually a sign of a weakened immune system I am completely freking out, have you ever heard if anyone that has caught hiv from just oral sex? Or maybe it was the fingering, and touching my penis after? 

5) If My results for hiv come out negative tomorrow it would have been 11 days post the two oral sex encounters, would that attribute to anything, being a 4th generation, maybe even at least a 50% chance I am not infected? 

6) I was thinking of taking pep for these incidents, but I remember reading on other posts Dr Hook , and Hhh stating that they wouldn’t even recommend it for oral sex encounters, even if the person wete positive, is this true? Do u agree with that? 

7) These all sound so atypical of ars , I am freking out so bad, 2 weeks later, swollen glands, sore throat, very congested, my head feels hot, may have caught a fever, If I hadn’t taken Ibuprofen, may possibly still catch it im not sure, what else could it be if not hiv?

I forgot to mention that my tongue is also white, and the lymph node under my jaw, I had never ever felt that one before, it feels bigger than a walnut

I would agree with Drs. Hook and Handsfield that oral sex is not a real risk for transmission of HIV.  The spots could be a rash from Bactrim, yes, and I'm a bit unclear about why this particular drug was chosen but you are likely finished with it by now, yes?  Your symptoms do not sound like HIV infection but if you are worried, you should talk with your own health care provider about your symptoms and about when it is appropriate to test next.  Sometimes it is hard to remember that when we have close body contact with someone else, lots of viruses, like the virus that causes colds and flu, can be transmitted.  I hope you will seek our your medical provider to discuss your concerns and evaluate symptoms that you feel are unusual for you.  We really cannot diagnose what might be going on with you over the internet but suffice to say I do not think for a moment that you contract HIV through this encounter.
