[Question #6274] Potential Exposure - Very Worried

17 days ago I made the horrible mistake of meeting with a CSW. This is the only time in my life I have ever done this and I have no idea what I was thinking. She was found online, I believe Korean, and cost around $170 for an hour and based at a hotel. I wouldn’t say high class but not a corner girl. I received unprotected oral for maybe a minute or two (was not expecting this and when I came to my senses on what was happening, I stopped it). We then had protected sex for maybe 4 or 5 minutes.  Astro-Glide was used as she seemed like she might have been sore and squirted it all over her vagina. She did have a full bush of pubes as well. Before the oral she wiped me down with a wet wipe and after intercourse she removed the condom and I wiped myself down with another wet wipe. I went to the restroom to wash my hands and make myself pee. I then ran home and showered and scrubbed myself to try and feel clean. Approx 3 days after this happened, I began to get canker sores on my gums and started to feel sick. By day 4 my gums were on fire and pretty much had canker sores all over my gums and still felt ill. Chills, lethargic and just over all felt bad. We did not kiss or anything like that so not sure how this might be related. I started using Peroxyl mouthwash and within a week, maybe a little longer, they went away. Around day 5 post exposure I noticed what appeared to be a small possible blister approx 2" north east of my penis in pubic region. I let it be for a few days before I eventually squeezed it. It bled for a bit then stopped. Within a week of popping it, it too started to heal (its day 17 can I can still see it healing but faint and not bad). Day 9 I was so freaked out that I went and received a standard STD panel. It came back negative for Chlamydia, Gonorrhea, Syphilis, Herpes1 and 2 as well as HIV 1/2 4th Gen. I have been worrying myself sick over this as I am married and do not want to risk giving my wife anything because of my stupid mistake. We have not had sex since this happened.

Is there any chance that this event caused the cankers on my gums and the one blister in my genital region? Again, gums were on fire and cankers all along gum base. Any chance this is / was herpes?

Should I trust the test results I received? From what I read that was too early for Syphilis. I’ve been worried sick that a chancre will show up in the near future and I’ll have to deal with that as well.

It has started to burn a little when I pee but that might be mental as I’m paranoid that something is going to show up. I was thinking of going and getting tested again at the end of this week (3 weeks post exposure, another standard panel). Is this too soon?

Odds of Syphilis showing up (this has been a very big fear).

What are the odds anything on my test at 9 days was wrong?

Possibility that I have NGU?

For me I know this was a horrible mistake and I will carry that with me I know. I just want to get over this fear and back to normal.

Forgot to add my testicles have been a little sore. Maybe this is from not having sex since this happened. I'd like to be able to get back to having sex with my wife. Would you think this is safe?

Welcome to our Forum. thanks for your confidence in our service.  I think you are worrying far more than is necessary and the odds that you were infected with anything by the exposure you describe is virtually zero.  Most CSWs do not have STIs and tend to do a far better job of being sure that they do not have an STI than persons who might be picked up in a bar.  Further, the higher the price, in general, the lower the risk,  My sense from your description is that you have become hyper aware and are noticing things and sensations that you would otherwise ignore.  As for your specific questions:

No, canker sores, which you have described perfectly are NOT STIS.  The seem to sometime occur with anxiety.  HSV does not involve the gums as you describe, not to mention that since you did not kiss, there is no way for you to get HSV from this exposure. 

2.  Should I trust the test results I received? From what I read that was too early for Syphilis. I’ve been worried sick that a chancre will show up in the near future and I’ll have to deal with that as well.

Yes. Technically your test for syphilis was performed earlier than is preferred.  A test at 1 month/4 weeks would be more reliable.  OTOH, your risk for having gotten syphilis is very, very low.  Few CSWs have syphilis particularly CSWs in the price range you describe.  While syphilis rates have increased of late, syphilis occurs primarily among men with other men as sex partners (~60-75%).  Further, your genital contact was condom protected and amongst heterosexuals transmission of syphilis through oral sex is extraordinarily rare.

You are overthinking this.  I recommend you believe your test results and stop the "crotch watch".  The testing you had is reliable and should be believed.  The test results are not going to change.

A very small fraction of 1%- likely less than 0.01%

Close to zero

No

Thanks Dr. Hook, appreciate the quick reply and insight. Based on your response I'm guessing I should feel comfortable and confident having unprotected sex with my wife again at this point?  Also, is it true that syphilis typically shows up at the head of the penis and not the base? I'll try to stop the "crotch watch" as you stated and move on. Again thanks for your insight and advice.

Correct, more than 85% of syphilis chancres appear towards the head of the penis and lesions are vanishingly rare in the pubic region. 

Also correct that there is no reason to abstain from unprotected sex with your wife at this time.  EWH
