[Question #6504] Recurrences and safe sex

I am redoing my question since it doesn't seem to have come through. I am 71 and have had herpes genitalis for about 45 years. I have been lucky to have married and my husband never got symptoms. We had children, but most of the time used condoms. I had recurrences every few months but they weren't too bad, and I didn't get on meds until 5 years ago. My husband passed away 7 years ago, and for the past 5 years acyclovir worked pretty well. However I read about kidney issues and my kidney tests seem to be slowly getting worse, though still normal. My primary doc thinks the risk is mostly from IV acyclovir but I am concerned, so I thought I would try going off. Within a month I have had one of the worst outbreaks ever. Over 2 weeks ago it started between my buttocks, then another one nearby, then 5 days ago two outbreaks on my right buttocks. So one question is why is this recurrence so bad, and is there hope of things settling back to where they were before I took acyclovir? 
Another question is because I am now in a relationship with a woman and want to be safe, on meds or not. I know that barriers are important. I would not have sex during an outbreak and then the rest of the time l would be happy wearing underpants, and even more clothing if necessary. So wondering whether one or more layers of clothing is an adequate barrier between genitals and hands. I also have questions about renal effects of oral acyclovir, and long term use of famcyclovir, which doesn't seem to be studied as much.

So there are very specific guidelines about how to dose people suppressively who have specific  kidney test values.    You or they should look this up so it can set your mind at rest OR change our suppressive dose to fit these guidelines, if necessary.  You may not need to adjust at all.  You or your doc should look it up online or on PDR. 
In terms of your new partner - underwear is plenty of barrier between you and this partner. 
Whatever applies to Valtrex or acyclovir would also apply to Famvir.

Thank you for this information. I am still considering a longer time off herpes medications because I went for many decades without them. Because acyclovir has reports of kidney damage (perhaps mostly from IV use?)  I worry that it could be a factor in my slightly worsening kidney tests. I also dislike taking medications. But I would like to learn your opinion of the risks of long term use of herpes medications.

Excellent studies have NOT found damage from long term use of these medicines.  I have had patients on these drugs for 30 years, without concern.  I don't really know what you mean by worsening kidney function tests but this is something to be discussed with your physician.  Regular testing of kidney function is NOT recommended in people on long term suppression.
